A stab wound to the neck is most likely a death sentence in resource poor areas
Nigeria is a resource poor area. This is true
The flow through the carotid artery, just at the side of the neck is roughly about 400mls every minute
The average volume of blood in a person is about 5 litres.
A person would bleed out in minutes. If you set a line and put a drip in, you're not replacing the blood, you're just replacing the volume in the vessels. You have to know what to do immediately to stop the blood loss
Most private hospitals in Lagos are not equipped for major trauma like this, hospitals are in levels, the highest levels are the teaching hospitals in a country
You'd need vascular surgeons,(people who repair vessels) cardiothoracic, general etc, you'd need a whole team
You'd need blood and fast! Lots of it. Most private hospitals don't have blood stocked, they have to request for them.
A well equipped hospital or emergency centre will have all these
I'm not trying to exonerate anyone, I just want you to know how easy it is to die here.
You can't just stitch up a blood vessel, that blood vessel is one of two that sends blood to the brain. It's also elastic which means that it retracts
It's goes "inside" when you cut.
You have to look for it. In bloody settings I must tell you that vessels are hard to find
You get a line set, you have to invite these surgeons or what you did was nothing. How long before they arrive. Remember blood is still being lost.
Replacing with fluid is not much if you don't get actual blood.
Where are all these surgeons in one place? A Tertiary hospital
How long did she stay before getting to the hospital? We don't know
So, we don't know how much blood she must have lost. Chances are getting dimmer by the second.
Every second counts!
In better healthcare centres, you call the ambulance, they come to the victim,
Quick assessment is done and they know exactly where to send her. The hospital is informed before hand and everyone needed is contacted and waiting. Theatre straight!
On the way, first aid is given
Patient comes in! Further assessment is done and patient is wheeled to the theatre ASAP!
The average Nigerian has lost faith in the tertiary health hospitals so we send to private hospitals where they may equipped to handle a knife stab to anywhere except, chest, neck or abdomen
What do you think expect?
There's not much they can do. So referral is one way to save a patient's life.
If you keep a patient when you should have referred, you're likely culpable in that person's death.
Save a life is the key thing here.
It hasn't dawned on us yet how moribund our health sector is. We keep shouting #FundMyHealth2020 but Nigerians think the doctors are fighting for their pockets.
Give us the tools we need to do our jobs.
You can't fight with just catapults! Most times we're handicapped
The system is broken.
Let's not even go to the issue of police reports. That no longer exists. The president assented to an agreement that basically states, "Treat first, report later"
We need more in the health sector.
It could be you, it could be me.
Demand better healthcare
I hate that when things like this happen, wmit becomes a battle of people against doctors. It's not. It should be us against the system! Because it can happen to anyone!
If you should get an accident on 3MB, are you getting to the hospital on time? Doctor or no doctor,
When you get there, will they have what it takes to save you?
These are the questions we should be asking! That's why the average Nigerian prays a lot. We get overwhelmed!
I say a prayer anytime I get on public transport, why? Baba God, na your hand I dey!
A better system
I have done my best to break it down into the simplest forms just so everyone gets it.
If you feel like dragging or roasting me, I understand and take it in good faith. It won't stop us from speaking. We need better. You and I

If you're unsure about a hair dye, apply a small portion on a part of your skin first to see if there's any reaction before applying on your scalp.
Some contain PPD (p-Phenylenediamine)
PPD is banned in some countries
It can cause this swelling
RT to help someone.
Not everybody will have this reaction to hair dyes containing PPD, but if you do, it can be really painful!
The reactions can start as soon as 2 days after
But that's not where it stops
You run the risk of getting sensitized to these dyes, what does it mean? It means that if you ever get exposed again, you may develop a life threatening reaction to it.
There is the case of a woman who died from a reaction to hair dye she had used before
I'll never forget this woman that came in with a swollen right leg, she had lost so much weight.
This was in 2015 where I was trying to get some skills before placement.
We asked questions, did our investigations and found out that she had cervical cancer, it broke our hearts. It had started spreading to other organs in her body and usually, at this point, her chances were looking bad.
She showed her previous photos, was so full of life
We talked a lot before she died, sometimes, when I wasn't on duty, I would go see her and spend time by her bedside, talked about her dreams, her passions, her kids had left her alone, her husband too.
I noticed people usually have clearer visions towards the end of life

The cough, sore throat and body pain you have right now is not malaria.
I know it's your body, but it's not malaria.
Here in this part of the world, we assume that once you have generic symptoms, it's malaria. So sometimes, without a test, people go out and get drugs.
Wrong.
Sometimes ,the symptoms are a pointer.
Cough, cattarh, are usually symptoms of a respiratory infection not malaria
Fever, joint pains, a feeling of being unwell. these are what we refer to as non-specific symptoms. Could be anything really. It's always best to test. See a doctor. Get your symptoms analyzed, it could point to where it's coming from.
Her mum was trying to force feed her pap. According to her, "E too dey select food"
Child is struggling to breathe, doctors are battling to save her.
I'll explain in pidgin so you can understand what goes on in force feeding.

You can't talk and swallow at the same time because the lungs and the stomach chook leg inside one trouser for your neck.
When you want to swallow, the gateman of the trachea, (the expressway that leads to the lungs) comes to close the gate so that food no go enter the lungs
Because if food enter, na wahala be that o
The lungs exchange gases, they don't digest food. It's not multipurpose lungs. Don't force them to do another work like our nation is doing.
When food or liquid find their way into the lungs, the lungs will fight it...na foreigner
