My cousin says from tomorrow in Greece if anyone wants to go out of their home - to the supermarket, pharmacy or doctor for example - they will have to fill out and sign this form explaining reasons why they need to go out:

There is a separate form for those who have to work to fill out.
My cousin in Greece tells me that from tomorrow they can text a number to fill out details of where they need to go and the reason. She says they then have to wait for an answer confirming it’s ok before going. When out they need to carry ID or passport.
