A hospital trust published guidance for staff which misdirected them on how to fill out death certificates - made it sound like was optional to include Covid-19 where doctor believed the patient had it, and that coroners should only be notified in “exceptional circumstances” /1

The worrying thing about the guidance was that it meant it was far less likely a doctor following it would include COVID-19 as an causative factor which is contrary to the ONS and NHS guidance. /3
