(1/9) #Physicalexam #Tweetorial @VCU_IMRes Cardiac auscultation is all about identifying normal from abnormal sounds. Like any skill this takes practice. In 1904 Dr Charles W. Larned published how to “mimic” heart sounds
Here is how to practice without a patient or even a heart!
(2/9) Set Up: While listening to your stethoscope, take the diaphragm in the palm of your hand and grasp it firmly
The following runs through how to make some basic cardiac sounds. Sounds where recorded with @thinklabs electronic stethoscope – no hearts used! #meded #medtwitter
(3/9) Normal Heart sound: Tap a simple One-Two tap with the first and second fingers – you just made S1 & S2! @audreyBJernigan @chrisyoungmd @DrJaredDyer @JCHillyard @kywaybill @TedRader4MD @fsmalick @Stern_bergler @RabiaQaiser3
(4/9) S3: add this extra heart sound by tapping second finger twice @vcuhospitalmed @RehanQayyum5 @ReenaHem @paletta @khess567 @ETKirtonDavis @ravichoxi @AbhiNandanMD
(5/9) S4: add this extra heart sounds by tapping first finger twice @mitchbk1 @IreneBFoley @alan_dow @cathymarcelo_md @CarolMarceloMD @Grebdlog2011 @bqueenhoo @gmacmommy
(6/9) Murmurs! Holosystolic murmur can be mimicked by dragging your finger across the back of your hand – amazing! @scall1200 @davidjessee @rvawellmd @lizzyvarnell @mia_bahagry @ErynnLayman @cderekleiner
(7/9) Aortic Stenosis: trace a circle over the back of your hand – you get the crescendo-decrescendo sound – cool! @abouzaki_md @justin_heizer @KSangal918 @kalahasty @jaideeppatelmd @AjayPMD @JCook2035 @GuruKowlgi @pranav_mankad @CoryRTrankle @ifarooqMD
(8/9) I use these tips even to this day to better “prime” my brain when investigating abnormal heart sounds – also serves as a great tool to teach normal and abnormal sounds #bedsidelearning - Regarding the skill involved – Dr Larned said this:
(9/9) Thanks to @FFulco7 for teaching me this trick many years ago – thanks to @dromron & @Sullydoc101 for help with recording! This is by far one of my most favorite things to teach - Enjoy!
