Richard Hayes Phillips unearthed this in his investigation into the Ohio 2004 election, documented in his 2008 book, Witness to a Crime: A Citizens’ Audit of an American Election. 2/
From p. 111, where Triad Governmental Systems, Inc. is noted as “an Ohio firm that created and maintains the vote-counting software in 41 Ohio counties”:
"Finally, our investigative team has learned that Triad Governmental Systems, Inc. not only provided the voting machines, 3/
[still quoting] the tabulating software, and the programs, but also provided the election results themselves to at least two Ohio counties -- Pike and Madison counties. We learned this when we asked to obtain the precinct canvass results for dozens of Ohio counties. 4/
[still quoting] Most Boards of Elections provided the results by fax, snail mail, or e-mail attachment, if they provided them at all. The Boards of Elections in Pike and Madison counties simply forwarded to us the e-mail attachments they had received from Triad, 5/
[still quoting] along with an identically worded cover letter from Brandon Sandlin, an example of which is provided here: 'Hello to all in Madison County! Attached, you will find the cumulative report with over and under votes reported as well as the official abstract. 6/
[still quoting] These reports may be printed for your records and then mailed to the state along with your other certification reports. You will also find the electronic abstract contained in the email. You may forward this email to the state 7/

And from pp. 110-111: “Representative John Conyers, Jr. of Michigan, the ranking Democratic member of the House Judiciary Committee, has asked the FBI and a county prosecutor in Ohio for a criminal investigation of ‘inappropriate and likely illegal election tampering.’ 9/
[still quoting]The initial request was based largely on a sworn affidavit from Sherole Eaton, Deputy Director of Elections for Hocking County. Ms Eaton says that a representative of Triad Governmental Systems, an Ohio firm that created and maintains the vote-counting software 10/
