2 - The 1st order said you could not enter the US unless you had a valid visa on the effective date. It's now changed to say it applies if you did not have a valid visa in one of the affected categories.
3 - What this is doing is saying if you had, for example, a B-2 visitor visa, you might be able to claim not to be subject to the order and could then apply for an H-1B or an L-2, for example. That's out now. I don't think it would have helped all that many people, IMHO.
