Example 1: 71 y/o male had fall on 4/6 that caused a C5-C7 quadriplegia; family complained of "altered mental status," "worsening confusion," and multiple falls at home between 7/28-29. Tested positive for COVID-19 at hospital 7/29; died late that evening. (4/8)
Example 2: 79 y/o female; asymptomatic for C19 but had +test in ER; went for lower extremity pain, diarrhea, and inability to ambulate for several days; "extensive medical history" with 13 contributory factors included congestive heart failure & kidney failure (5/8)
Example 3: 84 y/o woman in hospital since early June for L1 compression fracture & acute kidney injury; discharged mid-July & readmitted on 7/24 with thrush, vaginal yeast infection, leukopenia, & malnutrition; asymptomatic, but +test in ER on 7/24; passed on 7/29 (6/8)
Example 4: 85 y/o woman admitted for "debridement of worsening necrotic heel and sacral wounds," +test for COVID-19 the next day; died 2 days later with COVID as primary cause; had 6 contributory factors listed (7/8)
Example 5: 50 y/o diabetic man with no COVID-like symptoms; called 911 for chest pain; cardiac arrest on way to hospital; had "acute renal failure and a CT of the head revealed severe cerebral edema;" had +test in ER and died 2 days later (8/8)
