Shoulder surgery hurts!

Interscalene block is currently the gold standard

Most people start from supraclavicular fossa and then slide the probe up the neck.

Key is to identify the Cervical Transverse Processes to identify Nerve roots.

Also identify phrenic if possible

One of the potential advantages of the out-of-plane interscalene block is avoidance of the dorsal scapular nerve within Middle Scalene Muscle.

Make sure you do your shoulder block before the Suprascapular Nerve has left the superior trunk.

This is why traditionally supraclavicular brachial plexus block was not recommended for shoulder surgery

Biggest complaint I have had from pts having shoulder surgery with blocks is duration of analgesia Vs Heaviness!
I use superior trunk block with 7-11 cc’s of LA, and couple of cc’s to Supraclavicular nerves.
Usually spares hand.
How about u guys?

Short acting block with local infiltration?

Pre-emptive strong opioids to prevent rebound pain? 🙉

@docmorne will often add a PEC1 (interpectoral injection) for shoulder arthroplasty to get the lateral pectoral nerve!

I love regional anaesthesia and working with surgeons, & am also sad that I have finished Schitt’s Creek. Here is a thread of experiences as a regional anaesthetist..

The reaction of a new surgeon the first time you state you’d like to site a nerve block... for a trauma patient
The reaction of the anaesthetist when the surgeon states that blocks will slow the list down, and “The operation isn’t that painful anyway”
The activity in the anaesthetic room when we get the “go ahead” to put a nerve block in a patient- “as long as it’s quick and it works”

The concept came from an excellent editorial from @LloydTurbitt , @EMARIANOMD & @elboghdadly

One of the key points of the editorial was to focus on a small number of high-value blocks, and to train everyone in them, and then implement into pathways👇🏼
