Happening now. Hoping to hear from all the vaccine-related efforts from rheumatology researchers, clinicians, patients, and other stakeholders.
Review on vaccine responses among people with rheumatic disease, and what's known about the COVID19 vaccines thus far, from Joel Ernst (UCSF)
➡️schematic to remind us of cytokines important in vaccine Ab responses (same ones targeted by rheum meds)
Most vaccine response data from RA.
PPSV23, influenza better studied
HAV vaccine: uncontrolled study, but seroconversion shown by MTX, TNFi, TNFi+MTX therapy
This study had a control group, and looked at influenza vacc (from 2010). From this one we infer that TNFi therapy doesn't have much of a response on the influenza vacc response
schematic for the mRNA COVID19 vaccines
important points to consider regarding COVID19 vaccines and pts with rheumatic disease
Planned studies (starting as soon as Jan 2021) discussed:
⭐️@alhkim for COVARiPAD, a multidisc collaboration at WashU (rheum, IBD, uveitis, MS/NMO), to study C19 vacc responses in these populations vs healthy controls.
1* aim to determine magnitude & quality of Ab response
⭐️Mariana Kaplan for @NIH_NIAMS, a natural hx study of pts with Ab associated systemic autoimmune disease (eg RA, SLE, myositis, vasculitis), before and after C19 infection or vaccination
Plans to look at:
vascular inflammation on imaging
PROs, QoL measures
⭐️LUCIN study - Drs Kalunian, Pascual, Dall'Era in collab w @LupusOrg . Open label prospective study in SLE pts to eval immunologic (and clinical) response to 3 diff vaccine platforms
⭐️COVER-Rheumatology led by @RADoctor & Kevin Winthrop. To study C19 vaccine effects (lab based efficacy; safety & tolerability) among pts with rheumatic disease.
Holding underlying rheum therapies (eg MTX): proposed as an ancillary nested study
