A thread on what MOH is planning for people who got 1 dose of AstraZeneca (Covishield) and are waiting for their second. On May 9, Dr Deepa Gamage of the Epid Unit presented at "The Next Pandemic - International Conference" organised by the Medical Students of the Rotaract Club.
Three points
1. They are looking to get AstraZeneca doses from countries that may have doses they can part with.
2. There are studies overseas investigating the ability to use a different brand as the second dose - if good results, we would also have that option.
3. If all else fails, and no dose two after 6 months (hopeful we won't get to this), may possibly vaccinate with 2 doses of an alternative brand. They expect reasonable protection up to 6 months from the first dose.
NOT an official statement from MOH, more a thought process.

1/ It's gloomy news today. There are many plusses to our COVID response, and we must not lose sight of those. It's been a tough time on many - those involved in the response, and to the public. Testing numbers are really holding as back. Some thoughts:
2/ Great strengths with LKA's response, albeit with huge challenges and consequences:
ability to contact trace nearly all cases (this requires a good public health system),
tightening up boarders quite early, and
strict quarantine for cases,
which led to a smaller first wave.
3/ So our cases and deaths are far less than what we are seeing with horrific numbers coming out of the US/Europe. We are set up with the chance to really crush COVID and get back to some "normalcy", but to do this soon, we need to increase testing to detect new cases quickly.

In my ideal world, we would increase testing capacity enough to 1. test all people who come to hospitals with COVID symptoms (fever OR respiratory symptoms), throughout #lka. With further expansion 2. We would ask people in the community with symptoms to come and get tested. 1/4
It's "targeted" to find clusters earlier, rather than when there are 1000s of people and we stumble upon it from the current surveillance testing that is done (it appears this cluster was (thankfully) detected by such surveillance testing). 2/4
The point is, it potentially could have picked up much earlier if we had expanded testing, as there are reports that people related to the cluster were having symptoms even in late September. Early detection limits spread and hence limits the size of the cluster. 3/4

We all know about face masks, hand washing and distancing. I want to highlight high risk situations, so you can think twice when you face situations in the coming weeks.

1. Closed rooms with recirculating air (e.g. airconditioned, or no windows) are higher risk than larger rooms with free air flow, because virus aerosols accumulate and float around, and you're more likely to come into contact with it in higher doses. elpais.com/infografias/20…
2. The longer you spend time with a COVID+ person, the more risk you are at. This could be in one go, or over multiple shorter contacts during the day. Rule of thumb is 15 minutes over 24 hours is a significant contact.
