Semua pasti gembira dgn tren kes COVID yg menurun
Tetapi ini mesti diimbangi dgn ujian COVID yg mencukupi
Ini penting utk mempastikan kita mengesan bilangan kes COVID setepat mungkin
Yaitu kita tidak under-diagnos bilangan kes dinegara kita
Utk mengetahui kita telah mengawal pandemik dgn baik, kadar ujian positif COVID mesti <5%
Kadar positif ini ditentukan oleh WHO
Rajah menunjukkan bahawa kita gagal utk mencapai kadar positif ini
Maknanya masih banyak kes didalam negara kita yg tidak dikesan dgn ujian massa COVID
Kita boleh anggarkan kes harian COVID dgn kaedah berikut
Kadar kematian diMsia =0.56% [ (3536 kematian/627652 kes) X 100% ]
Maka jika pada 8 Jun 2021 kita rekod 76 kematian, maknanya bilangan kes COVID sepatutnya 76/0.56 X 100 iaitu 13,571.
Tetapi kita hanya merekod 5,566 kes
Respon FTTIS masih tidak pantas & cekap!
KKM hanya guna RTK-Ag sebagai ujian diagnostik dikawasan yang kadar positifnya >10%
Ini kadar WHO yg lapuk sebelum May 2020
Dikawasan yg kadar positifnya <10% KKM masih mewajibkan ujian PCR
Ini bertentangan dengan nasihat Expert Advisory Group (EAG) seperti didlm rajah berikut!
Kedegilan KKM ini akan melambatkan proses Isolasi, Kesan Kontak & usaha memecahkan rantaian transmisi COVID
KKM mesti ambil peluang MCO3.0 utk buat ujian yg meluas dgn program FTTIS yg pantas & cekap.
Jika tidak, kita akan terperangkap dgn kes yg bakal melambung apabila MCO dilonggarkan.
Seperti yg berlaku selepas MCO1.0 (18Mar-4May 2020) dirangsang oleh PRN Sabah &
MCO2.0 (13Jan-5Mar)
Saya hanya khuatir: 1. Pimpinan KKM masih guna kaedah FTTIS yg lembab 2. Masih guna piawaian WHO yg lapuk 3. Pimpinan KKM kini tidak berwibawa 4. Atau mereka dah burnt out 5. Atau agak ego, degil & tidak mendengar nasihat pakar lain seperti EAG

✅Terlalu muda untuk vaksin (un-vaccinated) ✅Tak cukup dos vaksin untuk umur (under-vaccinated)
Solusi untuk ⬇️ kes dan kematian Pertussis:
✅Vaksin Tdap (Tetanus-Diphteria-Pertussis) ibu hamil utk melindungi bayi
✅Vaksin 6 serangkai (termasuk Pertussis) 2,3,5,18 bulan yang pertama
✅Vaksin tambahan untuk remaja dan dewasa

The issues in the MOH are
✅Chronic maybe 10-20 yrs
✅With an acute on chronic crisis
✅Due to the acts of omission or commission
✅Mainly by technocrats warming seats in Putrajaya
✅Who marginalised the best & bold brains in MOH who dared to open their mouths
✅But of course the jurassic political stewardship is like hopeless with no roadmap for the future of healthcare

I have some insight into the problem though not as great @adeeba@ummuafeera whose passion & dedication I totally admire & respect!
✅Unfortunately, many who dabble in the PrEP issue have virtually no hands on experience in dealing with this most painful, sensitive & explosive issue.
✅They lack empathy and more crucially, they lack rahmah, compassion & love for the marginalised communities
✅They stigmatise further these vulnerable groups mainly young Muslims, instead of engaging with them to decrease their risky behavior & saving them from being infected with HIV/AIDS
✅You don’t achieve abstinence with one turn-over camp or 1-2 sessions of dr consult!
