🧵 on masks, vaccines and gathering in the age of COVID. There is so much confusion in the general public and even docs in the face of changing guidelines and all the misinformation out there, balanced with our desire for normalcy. I get asked often, so here is how I do it. 1/
I fully admit, it may not be perfect, but I have assessed the risks to myself and my family, and I am comfortable with this plan. We are all triple vaxxed and i have had a mild case of COVID. 2/
Situation 1- stores, indoors with mixed or unknown vax status, strangers. Mask up. Is it REALLY protecting me? Probably not. But I’m potentially protecting unvaxxed or under-vaxxed others, so I’m good with that. 3/
Situation 2- in a group of friends or family with 100% full vaccination. We live normal lives. This is the goal. This is the POINT. We have frightened some into thinking that this is bad behavior and it is NOT. our case rate is never going to zero. Too late for that. 4/
Our goal is no longer elimination, it’s likely too late for that, and maybe always was. We are trying to minimize suffering, death and over-utilization of healthcare resources. My 100% vaxxed group gathering is not going to meaningfully contribute to any of those things. 5/
Much of the resistance to mitigation strategies seems to be coming out of the frustration that even if we shut down things, even if we get the vaccine, that we are NEVER going back to a semblance of normalcy, so they think “What’s the point?” 6/
That is not my approach. I sell the vaccine as a solution to a terrible problem. Which it is. It lets you kiss your mom, hug your sister, sit inside and smile at each other again without fear of murdering them by accident. 7/
There are those who think that this is too risky. That any case is too many. That a group of fully vaxxed people may be drivers of variants, though the virus burns unchecked through the unvaccinated populace. To those, I would say…chill the hell out. We aren’t the problem. 8/
We have a new disease. It will likely be with us in some form forever. The further down the road towards having either vaccine induced or infection induced immunity, the more the hospitalizations and death curves will decouple from the case curve. This is good and necessary. 9/
Now, before you come at me, realize that I am assessing the risk of ME and MY family. Not yours. You may have a relative or friend with cancer. Or immune suppression. Those risks should be weighed in context and appropriate protective measures should be taken. For YOUR group. 10/
But if you are person out there in healthcare or not, who is telling people that masks, distancing and LIFE should not change at all even in a group of healthy vaccinated people, I submit that you may not be helping, and in fact may be driving people away. 11/
This ran long. Sorry I’m wordy. In summary- get your freaking vaccine, wear your masks in public spaces, don’t lick doorknobs, and if you want to hang out with a group of fully vaccinated low risk people without masks, go nuts. I’ve got your back. ~fin

🧵 Hi. I’m an ER doctor. I’m vaccinated, but 10 months out with a high level of daily exposure to an airborne disease. I can’t get most patients to keep a mask on when I’m out of the room,or wear it properly when I’m in. I now have Covid. Mild for now. 1/
And hopefully, gonna stay that way. I am very optimistic that I will stay well, thanks to the vaccine. My body is already at war with this stupid bug, beating it back. If I had gotten this last year, I would have been very scared. I’m middle aged, and smoked for 25 years. 2/
My cousin almost died early in, before the world closed down. I am prone to health related anxiety at times, anyway. But now I feel irritable, not anxious. Could I get really really sick? Sure. Could I even die? Yep. But I’ve done the right thing to minimize those chances. 3/
I see a shocking amount of vaguely weak elderly folks with decreased appetite but nothing else particularly wrong in the ER. Holding a hand and asking the right questions with empathy will often elicit tales of the overwhelming sadness of loss or fear of their own end. Short 🧵
It is easy for us to spot the anxious or depressed youth, the histrionic middle aged person, the substance use disorders. But so often the mental health issues in our elderly are brushed aside, written off as a result of the chronically “dirty” urine that is asymptomatic… 2/
Maybe they are thrown on Zoloft, or given trazodone for sleep. They are encouraged to go to a nursing home. They bounce to and from the ER, their primary care, and back again, frustrated because all their labs are good, but they still feel like dogshit. 3/
Imagine being privileged enough to have been born into a situation that allowed for a stable income with no food insecurity, free of the scourge of addiction, being provided a good education and arguing that paying people 7.25 an hour is enough. 1/

I have found that in times of political confusion, particularly when emotions are running high and creating tunnel vision, THE PRESENCE OF NAZIS can be an extremely helpful indicator.
If I am attending a local demonstration or event and I see Nazis…neo-Nazis, casual Nazis, 2/x
master race Nazis, or the latest-whatever-uber-mythology-Nazis, I figure out which side they are on.
And if they are on my side of the demonstration? I am on the wrong side.
It is tough to argue moral equivalence when I am standing next to a Nazi. 3/

College 2020. My daughter went to school, moved into sorority house. Next day, many girls go to frat party (not my kid). Within 4-5 days she has to quarantine back here. Within 3-4 more days, her whole house is on quarantine. 1/
She is banned from campus due to contacts. We move her into an apt off campus with 3 other negative friends. Finally she gets to go back to campus for her ONE in person class. Day 1 AFTER class, a kid walks up to the prof and tells him that she has been feeling poorly. 2/
She has a headache, fatigue and has lost her sense of smell/taste. And oh- all her roommates have the same. Professor was like 👀. Of course she was at the closest desk to my kid. And yes, she tested positive. So now my daughter awaits her second ban from campus. 3/
