Yes, America has lost the plot. "On the ground, the U.S. is now running an uncontrolled experiment with every strategy all at once. COVID-19 policies differ wildly by state, county, university, workplace, and school district."
"Due to polarization.have also settled into the most illogical pattern possible: least vaccinated communities have some of the laxest restrictions, while highly vaccinated communities (most protected) tend to have some of the most aggressive measures aimed at driving down cases."
But I am seeing education/schools as a lightning rod issue for American parents and so perhaps the plot of the twilight zone occurring in highly vaccinated and protected communities will clarify around this
I have op-ed tomorrow: hopefully clarify goals; CDC putting restrictions like masks in July on outdated community transmission metrics which were relevant before vax (not after) put blue US in twilight zone w/ endemic virus, set up natural experiments in US
Reason Atlantic article is so good is that it points out that the CDC metrics on community transmission aren't the right ones (other ways to measure community burden that make sense after vax); these keep blue enclaves restricted as if to make up for red states; need unified plan

Entire Bay Area is back in CDC's orange and red tiers for COVID spread - right, but this is because a highly vaccinated place will always be at risk of being in these "tiers" due to the outdated definition of the CDC's metrics of community spread. sfchronicle.com/health/article…
To put it most simply, he current CDC guidance states that masks should be used in areas of high to substantial transmission (based on test positivity and cases/100,000 rates) but those metrics only made sense when mass asymptomatic testing was occurring prior to vaccination.
After vaccination, per the CDC guidance, only those with exposures or symptoms should test so tests are likely to reveal positive cases, which is not a good measure of community transmission or spread (but just means the people who should test are testing). Marin mask mandate
Wow, a powerful visual of how kids lost more school in the US than another wealthy nation. We here in blue CA were 50 out of 50 in the country in opening schools so we have a lot to make up now, including instituting test-to-stay: washingtonpost.com/outlook/2021/1…
A difficult slide from the CDC ACIP meeting today for 5-11 suicides as many -including @LeslieBienen and @JeanneNoble18 - were trying to get statistics on self-harm among children during CDC-supported US school closures. We wrote this piece about it. wsj.com/articles/the-p…
2020 statistics still pending to see if higher (was higher in CA if you read WSJ piece above). In Bay Area/ CA, most health officers have said adult masking ends 8 weeks after availability of 5-11 vax. Not contingent on children getting it, just available. To restore kid's lives
Some parents need more time; that time will be given. As Peter Marks from FDA says- he has a whole army of FDA experts evaluating safety before full approval. 10 microgram dose seems very safe; I will be vaccinating my 11 year old - 2 doses 8 weeks apart nytimes.com/2021/10/30/hea…
Biden could only mandate vax once FDA approved (Pfizer >16, Aug 23); kid mandates can't come until full approval (12-15 still under EUA since May 10) for parents to watch safety & mask rules. I encourage kid vax now, mandates only legal with FDA approval missionlocal.org/2021/10/area-d…
Remember, authors don't write headlines. This is why I am getting my child vaccinated and why I think is a good idea. This does NOT mean it will be mandated until FDA approves and that many parents need/want more time, which is justifiable. sfchronicle.com/opinion/openfo…
CDC plagued by confusing messaging, critics say - Late-stage epidemic in a country is a difficult time to communicate clearly but messaging lately ("winter surge will be same in 2021 as 2020") has been around acting like we don't have the vaccines news.yahoo.com/cdc-messaging-…
Anyone telling you 2021 is same as 2020 (on schools, holidays, etc in places with high vaccine access) doesn't understand immunology
Harder to get on top of misinformation (heard Facebook not doing that) but easier for the Biden administration to deliver more positive messaging on the vaccines
How other countries can teach the Bay Area about the COVID-19 endgame - Think important for California to keep looking at Denmark (opened up completely on Sept 10 at same vax rate) for their trends sfchronicle.com/opinion/openfo…
If you look at Denmark's graphics, you will see cases going up but deaths staying low - in fact, please go to this website & put your cursor on y-axis (0-4 per day since opened) graphics.reuters.com/world-coronavi…
So, keep on looking at hospitalizations as cases rising in highly vaccinated regions are not the best indictor of the high protection of the vaccines (can be simplified into T cells/B cells - high protection- vs antibodies - which fade; see pinned tweet)
ENDEMICITY: Singapore also makes announcement it will abandon COVID-zero strategy leaving only China currently pursuing this strategy worldwide. Vaccines have massively decoupled cases from hospitalizations & older patients should be boosted & protected npr.org/transcripts/10…
Somehow cases in light of vaccinations have led some to think vaccines don't work. It is the amazing ability of the vaccines to defang the virus that show they work.
We can see different states in the US also pursuing COVID zero too but it is the duty of public health to attend to all ills of society, people didn't come in because of COVID fear " we're seeing more acute cases than we were pre-pandemic" medscape.com/viewarticle/96…
