The explanation given here on condition of anonymity is that Psaki was talking about a different, forthcoming report.

To the extent that "[m]any people have wrongly conflated" the reports, as the anonymous official says, *that's on the White House*, not the reporter.
That annoys. I mean, I guess take your shot, but it sure doesn't seem like Psaki was talking about a forthcoming report when she said "it's important for people to understand that there was no after-action report."
