My first session for the last day of #ATS2022 is “Clinical Year in Review 4”!
First up we have Dr. Kimberley Haines (@HainesKimberley) with “Long COVID and Post-Intensive Care Syndrome.” #ATS2022
(Full disclosure I missed the first few minutes of this lecture.)
This study of looked at post-COVID hospitalization care delivery. #ATS2022
This was a study of long COVID in pediatric patients after hospitalization. #ATS2022
Discussion points. #ATS2022
Future research directions #ATS2022
Implications for practice #ATS2022
Next we have “ARDS” with Dr. Kusum Matthews. #ATS2022
Defining ARDS #ATS2022
We possibly should be considering redefining ARDS like this. #ATS2022
Latent class analysis can be used to understand heterogeneity of ARDS populations and presentations. #ATS2022
This was a LCA study in “Thorax.” #ATS2022
These model types were found to be clinically relevant! #ATS2022
This study looked at non-invasive ventilation and outcomes. #ATS2022
Fewer patients treated with CPAP had a lower odds of intubation and death. No significance difference was seen in HFNC (but underpowered). More adverse effects with CPAP. #ATS2022
These studies also found a positive effect of HFNC. #ATS2022
Summary points #ATS2022
Re-analysis of EPVent-2 (effect of esophageal pressure-guided PEEP) #ATS2022
Recap of the original study #ATS2022
Results of the re-analysis #ATS2022
Takeaways from this study #ATS2022
This trial looked at PaO2 targets of 60 vs 90. They found no difference in mortality of adverse effects or vent free days. HOWEVER, they used SpO2 for titration and they did not consider race or skin tone. #ARDS2022
This study analyzed discrepancies between SpO2 and PaO2 in patients with darker skin tones. Hidden hypoxia was associated with increased mortality and organ dysfunction! #ATS2022
Takeaways #ATS2022
Additional articles to consider reading #ARDS2022
Next up is my old friend Dr. Joe Cicenia (@handsnotmitts) with “Interventional Pulmonology.” #ATS2022
This study looked at COPD survival with bronchoscopic lung volume reduction. There was increased survival in treated patients. #ATS2022
This study found that bronchoscopic endobronchial valve treatment may decrease pneumothorax risk. #ATS2022
This study looked at bronchoscopy in patients with COVID. Most were done for diagnostic intent, and 25% were done for therapeutic reasons (mainly secretion clearance).
#ATS2022
This was a study from Spain of bronchoscopies in patients with COVID. A higher population in this study was for therapeutic intent. #ATS2022
This study looked at performance monitoring of EBUS programs. #ATS2022
This study looked at factors associated with the diagnostic accuracy of robotic bronchoscopy. #ATS2022
This study looked at shape-sensing robotic-assisted bronchoscopy in the diagnosis of pulmonary parenchymal lesions. #ATS2022
When you put these studies together you see that localization doesn’t always equal yield. #ATS2022
This RCT evaluated HFNC vs NC during pediatric bronchoscopy. HFNC had less hypoxia but no change in clinical outcomes.
#ATS2022
This study looked at the value of dyspnea and spirometry in detecting relapse of benign tracheal stenosis. The peak flow was highly correlated with the degree of stenosis in patients with relapse.
#ATS2022
Other studies to consider reviewing. #ATS2022
And to finish us off we have the phenomenal MedEd leader Dr. Stacey Kassutto (@SKassutto) with “Medical Education”! #ATS2022
This study of the diversity of medical students over 4 decades found the biggest change was the increase in women. #ATS2022
Study takeaways #ATS2022
This study looked at exposure to workplace trauma and risk of PTSD in interns. Over half of them experienced trauma and 1 in 10 screened positive for PTSD by the end of the year.
#ATS2022
Other findings from this study #ATS2022
Takeaways - note that this was done BEFORE COVID. 🤯 #ATS2022
This study looked at the perceived impact of COVID-19 on fellowship. #ATS2022
Study takeaways #ATS2022
This study looked at a machine learning-based decision support tool for residency applicant screening.
#ATS2022
Study findings #ATS2022
Study takeaways #ATS2022
This study looked at the relationship between IM residency and PCCM fellowship milestones.
#ATS2022
Study findings #ATS2022
Study takeaways #ATS2022
Thanks to our speakers for a great Clinical Year in Review session! #ATS2022

Travel Tips Thread!

Many of you have reached out, asking me what company I used to plan my trips. I actually plan the vast majority entirely myself. So since so many of you seem curious, here’s how I do it: 1/
Disclaimer 1: For certain polar locations I have booked cruises through Quark Expeditions. These include my trip to Antarctica, as well as upcoming trips to Greenland and Svalbard. I particularly like this company due to their focus on environmental protection and education. 2/
Disclaimer 2: I did use a travel agency for my most recent trip to Africa, because it is very difficult to book, safari and transfers there without someone with expert knowledge of the area. 3/

My final #SCCM2023 tweetucation session for today is “Late-Breaking Studies Affecting Patient Outcomes”!
First we have “Continued Enteral Nutrition Until Extubation Compared to Fasting Prior to Extubation in the Intensive Care Unit: A Clustered Randomized Trial” with Stephan Ehrmann (@stephanehrmann)! #SCCM2023
Nearly all ICUs impose some form of fasting before extubation, but this time seems to be decreasing. #SCCM2023
