A report on the state of emergency medicine.

It’s not good!

7 in 10 Clinical A&E Leads treating patients in corridors last week

“This is the height of summer and yet we are seeing a state of affairs that we’d be dismayed by even in the depths of winter.”
President RCEM.

As for the government’s plan:

“Little help for community health teams. Little funding. No timescales. No transparency. No accountability. No improvement. The ‘plan’ has comprehensively failed so far.”

Sounds familiar.

It’s tragic that we have to be so focused on a dysfunctional government, their various delinquencies, and their incompetence, as this lack of access to emergency care is one of the greatest threats to the British people.

Todays debate in parliament will not get much attention.
But, all roads lead to a completely useless and malaligned government.

As it stands, the greatest threat to the British People’s health is This Government. So we are right to focus on challenging the government and holding them to account.
People are dying who could be saved. Because this government refuses to support the NHS during its worst crisis.

The bigger question is why?

And as more info comes out about dodgy dealings with private providers, the reality sinks in…it’s simply about money.

There was plenty to be upset with in the PM's speech.

His assertion he did well with Covid was a shameless attempt to spin a failure into a success.

He must be accountable. But also, it is dangerous for our future to think the UK Gov response was good or even adequate

And to be clear this account is mine, as someone on the frontline since the start, and also involved in Covid research and policy development.

And it is not putting the UK down. Quite the opposite. We have the resources and grit to do so much better if we have better Leadership!
Comparing the 15 most populous high-income countries

The big number of total Covid deaths (where data are available) is here...

We didn't do as bad as Poland, The US, Chile, Italy, or Belgium.

But nearly double the deaths of Germany, Netherlands, Canada, and ten times Japan Image

We are all fed up with Covid, but it’s here, and to protect ourselves and our families we need to face it.

And it actually requires minimal effort to reduce the risks.

Let’s start with what to do if you get Covid…
🧵

There are many sources advising on what to do if you get Covid - you should refer to your local one.

Job 1 is not to change your health-seeking behaviour. That is, if you are worried then contact whom you would normally.

The vast majority of Covid, even in vulnerable patients, resolves without event. It can be like a cold or flu and you start feeling better after a few days...

If there is one lesson we should take from Covid so far, it’s that we live in an age of disinformation and political Spin. And it works!

Arguably, the ease in which we are manipulated is one of the greatest threats to humanity and social justice.

Covid is a good example…
🧵
At the point where the requirement to self-isolate if infected was removed in the U.K., over 70% of the public thought it should continue…
…and science and common sense backed this consensus. It makes no sense at all to permit those infected with Covid to spread it freely. Not on an individual risk, workforce, economic, or healthcare burden perspective.

But politicians were desperate for a win, they needed a win!

I will say it again, if you do not want to wait an hour for an ambulance, 12 hours in A&E, days to see a GP, months for your cancer treatment to start, or years for your operation, job 1 is to pay NHS staff properly.

We will not stay under these pay and conditions!
Since this Government came to power, doctors have taken a 4-9% pay cut, nurses a 7% pay cut.
We stayed when the pandemic hit. But we are now leaving in our droves…

Highest resignations in NHS history.

One of the first big questions with Covid was: would getting Covid provide lasting protection against getting Covid again?

Now we know that reinfection is sadly all too common, the next big question is: is getting reinfected a big deal?

Evidence suggests it can be...
It was a big kick in the teeth when we realised Covid is quite good at escaping our immunity and that our immune response to Covid gets poorer over time.

The promise of enduring "Herd Immunity" was, it seems, premature and maybe now quite counter-productive.
Whether the second, third, etc., infection still carries the potential for severe problems is a big question for policy-makers and the public... How much effort should we make to avoid Covid going forward, esp. given most of us have had it and are vaccinated (in rich countries)

Should you worry about the new Covid wave?

It is a worrying time:

> Third wave this year

> Still killing more than any other infection

> With the pressures on the NHS it’s not a good time to get Sick.

> Long Covid increasingly more a worry

But we know Covid better…
> We are much better at treating Covid, at least in the acute phase.

> We know just how safe and effective vaccines are at preventing severe disease and to some degree Long Covid

> ICUs are less overwhelmed with Covid and have expertise in managing the critical Covid patient
We also know a lot about preventing it

- Masks are much cheaper and reduce the chances you will get Covid (or pass it on) (FFP2!).

How much you wear a mask depends on circumstances. I wear one on public transport or when shopping. Of course, for health appointments. I also…
