I’ve been a hospital manager for >15 years - much of that time spent managing flow and urgent care - and it’s never been a harder or more dispiriting job.
But there are still some EASY things that will always improve flow if we focus on getting them right every day.
A short🧵
#1 - “EARLY”
Patients admitted before lunch stay in hospital ~1-2 days less than those admitted in the PM.
Patients discharged home in the day have more chance of staying there.
Getting people admitted EARLY and home EARLY is important
#2 ASSESSMENT
Admitting to hospital should always be a last resort so we have to make sure we have effective ASSESSMENT pathways in place, reliable senior decision making in ASSESSMENT areas and keep ASSESSMENT areas free for that purpose - avoid bedding down at all costs!
#3 STUCK
People in hospital >7 days are at higher risk of deconditioning and needing more care on discharge.
Putting in processes to ensure management visibility of what’s keeping people STUCK and prioritising activities targeted to get home quickly will help reduce delays.
#4 YOU
Creating the conditions to ensure patients get the care they need when they need it is a team sport. It takes everyone on the team to understand their role and the part they play to make it happen everyday.
YOU have a really important role to play in making it happen!
So even though things are tough right now for everyone working in the NHS there are some EASY steps we can take to make things better.
1. Getting people home EARLY 2. Having effective ASSESSMENT 3. Make sure no one gets STUCK 4. Understand what YOU can do to help
Let’s make a pact to work together to make it EASY for our patients to get the right care when they need it, go home when they’re ready and for our teams to know what a difference they make in making that happen.
