A follow on from @EBMgoneWILD discussion about the mess that is the 🇺🇸 medical system
Malpractice insurance is designed to trap physicians in jobs for life.
On a thread with fellow #obstetricians and discussing tail insurance. 1/
For those not familiar with the hot damn mess of American med mal, obstetricians have one of the highest malpractice premiums. It varies from state to state and increases every year.
About 18 years ago as a new grad, mine was a real cheap $55,000/year. 😒 2/
If you have a 'Claims Based' malpractice policy, you *have* to buy a 'Tail policy' if you want to leave your current job (unless your carrier covers your new job/state)
Now, the 'Tail Policy' cost varies based on how long you have been working. The longer = more expensive 3/
Currently I'm seeing policy quotes as low as $77,000 (Georgia) all the way up to $230,000 (Maryland)
Oh, and these have to be paid IN FULL up front. No payment plans 4/
So another way that doctors with additional financial constraints become trapped in positions. When you are 3-4 years out with $200k in student loan debt, kids and household bills, you don't have an extra $100k to give to an insurance company 5/
My recommendation for my younger colleagues:
-only consider jobs with Occurence policies OR
-negotiate the tail to be paid by your employer
Don't become trapped in a job not right for you

There are 328,200,000 people in the USA.
Black Americans make up about 13% of the population
There are currently about 877K practicing physicians in the USA....
4% of the 877K are Black
ONLY 2% of physicians in the USA are BLACK FEMALES.
We are out there, serving our communities, taking care of the sick, the disinfrachised. Listening. Doing. Acting.
We are tired. We are only 13% of the population but we are dying faster and worse from almost every disease/disorder...
We know it isn't just race that determines these outcomes. Its a long standing system of #bias #structuralracism and social determinants of health that make this an almost impossible battle to win....

We lost another doctor today
To COVID
But this doctor was mistreated. Her symptoms were ignored, downplayed and dismissed. She posted a heartbreaking video begging her fellow doctors to help save her...
She called the patient advocate...
She called the chief medical officer...
She was in pain and ignored
Fellow docs got her into another hospital...
It was too late..
She passed away today
The system failed her.
It failed us...
Anyone want to guess the race of this doctor?
I'm pissed cause she didnt have to die like that
Her pain was dismissed
She was discharged WITHOUT being stable and was readmitted to another hospital because she was too scared to go back to the first hospital...
