v2.0 with adjustments
Radicular pain and/or radiculopathy. A thread on decision making.

1⃣ Setting the scene

3Ps/serious pathology (red lines) aside. What does the evidence tells us?

✳️Wait & see
✳️Physiotherapy interventions
✳️Nerve root block
✳️Microdiscectomy
👇👇👇 Image
2⃣ A stepped care approach

So, who will get better anyway? What about relieving suffering? We only have early work into trajectories and prognostics.

Current guidelines and national pathways recommend a stepped approach. One that is 'least intrusive'.
👇👇👇 Image
3⃣ Management variables

What characteristics should we consider? To map onto the stepped care approach.

✳️Confidence in dx
✳️Prognostics
✳️Additional factors
4⃣ Confidence in dx

Does the story fit?
Is symptomology consistent?

Does the pt have clear muscle power loss? Any objective numbness with clear borders?
👇👇👇 Image
5⃣ Prognostics

Green: Positive prognostically
Red: Negative prognostically

Use these to plan care and have sensible conversations with pts.
👇👇👇 Image
6⃣Additional variables

Does your pt even want to consider interventional procedures ? Do they understand their options.

Gone are the days that pt MUST have interventions done to them.

What about surgical fitness?

So remember, a stepped approach with least level of intrusiveness. Remember your redlines.

A tweetorial on navigating the role of imaging in spinal care.

THE PROBLEM.

1-4 patients with LBP are referred for imaging
pubmed.ncbi.nlm.nih.gov/30760458/

Of 306 MSK-MRI requests only 4.9% were indicated and only 1.3% lead to change in management

This is despite normal scans not offering much reassurance, even when used to screen out serious pathology.

Intuitively, giving someone a problem without a solution e.g 'wear n tear' doesn't either.

Here is a tweetorial on bending and low back pain.
Bit of a longer one but a number of juicy papers included.
1/9
There is a long held belief that bending, particularly with a flexed spine is harmful and more likely to lead to low back pain compared to a more sparing type strategies.

Avoiding rounding the back is a common belief held by physiotherapists and manual handing advisors

Here is a thread on FMS. If you’re a generalist or a specialist you see pts with FMS (dx or not) and it matters. Here is some needs to knows. 1/8
FMS is a syndrome. Pts suffer with more than pain. Intrusive fatigue and a number of somatic problems have a big a impact on health related quality of life.

Here are the presentating features in a little more detail 2/8
FMS is not a fictional disorder. Frivolous labelling on one end and overmedicalization on the other have led to some believing its a micky mouse disorder while others miss vital information and support. 3/8
