When part-time is really full-time

When GP hours are counted

Official figures for GP working hours are counted in sessions

A session is 4hrs 10 mins or half a day

9 sessions is 37.5hrs or full-time officially

But there is a problem

🧵1 Image
A GP session or half day can actually exceed 6hours

For the majority it is between 5-6hrs

This means the hours counted are underestimated by 1-2 hours session

So GP hours are not officially accurate

2🧵 Image
GPs working 6-8 sessions are officially counted part-time

The vast majority are often exceeding Full-time hours

3🧵 Image
There are also safe recommendations for the number of consultations a day

The majority of GPs are exceeding these recommendations

And this does not include other work done

4🧵 ImageImage
All this is in the backdrop of GPs leaving

The UK is training GPs but the overall number is falling year on year

This means the workload is increasing year on year

5🧵 Image
Of major concern is that younger GPs are leaving in greater numbers

The retention crisis in General Practice is a major problem

6🧵 Image
The reasons are multiple for the GP crisis

But underlying it all is a lack of funding

A Contract that demands too much for too little

And 360,000,000 appts a year is still not enough

Half the population consults every month

There is a #GPCrisis Image

GP Crisis is being ignored despite the many reports

@wesstreeting even commissioned the Darzi Report but is ignoring it’s advice on GPs

Primary Care Training Hubs

Promoting the training & development of additional roles in General Practice, except GPs & Practice Nurses

So no prioritisation for tackling GP /GPN unemployment & underemployment? Where is the accountability? @wesstreeting

hee.nhs.uk/our-work/prima…Image
Whilst the Multi-disciplinary Team is vital to good community care, so is planning, joining up thinking & accountability

There has been a huge rise in additional roles without real understanding of the need for GPs & GPNs in practices

This is failure @NHSE_WTE & @NHSEngland Image

Training Hubs are focused on using First Contact practitioners

Undifferentiated (undiagnosed) patients

This is a role for GPs who have had a minimum of 10y to complete their training

There is no need for other clinical colleagues to take this role

It’s increasingly hard to see a Doctor

Policies made by NHS leaders & Governments have lead to fewer GPs

Many GPs finishing training next month will not have jobs to start, with many unemployed or working less than they would chose

All due to policies & lack of funding for GPs Image
The biggest change over the past 5 years has been NHS England led imposition of ARRS (Additional Roles)

Aimed at increasing access to a wider workforce in primary care

Until last year this excluded GPs & practice nurses resulting in fewer roles

But a massive expansion of ARRS Image
Complaints from Nurses & GPs were finally heard last year

NHS England was forced to open the scheme to GPs & Nurses

However there was a big problem, they restricted GP roles to those that had qualified in the previous 2 years

It was announced as an emergency measure but….. Image

Blackpool

Known for the Tower, the winter lights , summer holidays & Strictly Come Dancing. But behind the glamour is one of the poorest most deprived areas of the country

The life expectancy is more than 10yrs lower than the UK average & health inequalities are a huge problem Image
As recently as October 2024, the local health leaders Kevin Lavery & Dr Andy Knox were sat in front of NHS England’s board sharing insight, successes & challenges. They were applauded for the work with Chair Richard Meddings full of praise.

1hr 14mins Image
The glamour and glitz has faded. The Chair replaced & the CEO gone. The plaudits & praise forgotten in the harsh reality of the current NHS

Despite Wes Streeting’s promise of more funding for the community, top down approach to care is failing patients theguardian.com/society/2025/j…

The UK is a relatively low tax economy

This means the revenue raised by Government as a proportion of GDP has been low historically

Less funding raised for infrastructure/healthcare/education etc

A thread 🧵 Image
Infrastructure spending is some of the worst

This results in poorer roads, hospitals, less beds etc Image

Healthcare spending

It is often slated as high cost but in relation to GDP the government has to spend less than peer nations Image

GP TRAINING an example for @gmcuk @wesstreeting @DHSCgovuk @NHSE_WTE

GPs are trained extensively Minimum:
5 years of medical school
2 foundation years which include surgery and medicine
Plus 3 years minimum as experienced Doctors with extensive supervision, exams & assessments Image
GPs understand the full curriculum of education as set out by RCGP

This is a full and comprehensive curriculum covering all aspect of medical education and health & social care rcgp.org.uk/mrcgp-exams/gp…Image
The examinations GPs undertake are thorough & evidence based, with years of research & experience.

GPs have clear guidelines and recommendations regarding scope of practice, before they can consult with undifferentiated patients

Only GPs can do thisrcgp.org.uk/mrcgp-exams
