@MondayNightIBD @AaronBlocker_ Just to share my personal story, I found my biggest barrier was navigating the different care coordination style amidst a major life transition that coincided with my #PedsGI to adult GI transition! 1/5

@MondayNightIBD @AaronBlocker_ Not every patient will be prepared for transition-- definitely the goal but will not always happen.➡️ For me, my dad lost his job so we went on Covered CA. By the time we had new insurance, I turned 18, and had no choice but to abruptly transition. 2/5

@MondayNightIBD @AaronBlocker_ Had to now navigate being proactive with my care and more responsibility in decision-making ( not as intuitive as it seems when coming from Peds).
My advice to patients who are transitioning➡️prioritize learning different members of your GI team + build those relationships! 3/5

@MondayNightIBD @AaronBlocker_ Taking into consideration my cultural background, shared-decision making heavily involves family. ➡️ I was facing pressure from family wrt my treatment.
Trouble separating out what's best for me vs my family ➡️hard to participate in decision-making with my care team. 4/5

@MondayNightIBD @AaronBlocker_ My transition was during 1st year of college ➡️whole host of psychosocial problems. I didn't know some could be IBD-related (ex: disordered eating).
Adult GIs to be cognizant of unique situations that YAs may face at time of transition (mental health, substance use, etc.) 5/5
