I had the pleasure to present (virtually) at @winslow_la conference on Covid activism today. My talk "Canaries in the coal mines. Patient activism in early Covid epicenters" offered snapshots of survivors' advocacy leading to #LongCovid recognition

Pandemics are history making events. The way we record pandemic history has huge implications for public health, public safety, preparedness, prevention, justice, patient rights. A key historical aspect of this pandemic is patient activism, which redefined how Covid is understood
Covid survivors in the first wave are the canaries in the coal mines: often abandoned at home with no medical care, in collapsed healthcare systems, they/we raised the alarm about the true nature of Covid, and brought #LongCovid to light

With 7—30 million Covid deaths and 65—400 million estimated #LongCovid cases so far, the Covid pandemic is, in my opinion, one of the greatest health disasters in recent/recorded history, especially if we acknowledge the prodigies of modern medicine

#LongCovid is part of the history of this immense pandemic tragedy. But it's also a story of landmark collective, grassroots, international activism. Long Covid was named, identified and defined by patients themselves in 2020

#LongCovid is the patient made term for the long-term symptoms, signs and sequelae of SARS-CoV-2 infection. As a concept it focuses on prolonged multi-system disease and the prolonged disease process triggered and potentially driven by such infection
#LongCovid was actively contesting the presentation we received of Covid in early 2020 by many sources: a "short", "mild", "mostly respiratory disease" in many, especially the "young and healthy", who were to recover in 2-6 weeks maximum. Death was for the "old and frail"
Early reports of persistent symptoms, and of a Covid illness which wasn't "mild", "respiratory only" and "short", even in the "young"/"healthy", started to emerge from early epicenters by March—April 2020. These reports often came from Twitter and socials

To raise awareness of prolonged Covid illness and severe Covid even in those left home with no care, survivors made use of hashtags and patient made terms like #mildcovid (used ironically), "long haulers" (by Amy Watson), #apresJ20 (beyond the 20th day by @lapsyrevoltee)
@lapsyrevoltee In May 2020, reports of prolonged disease were multiplying. Prof Felicity Callard blew up the concept of "mild Covid" in @somatosphere (8 May). A team of patients in the US produced a survey of persistent symptoms (11 May). The name and hashtag #LongCovid coined on Twitter

@lapsyrevoltee @somatosphere By June—July 2020, #LongCovid was going viral and entering wider conversation in policymaking, the press, research. There was a dense process of collective knowledge building by patients on social media, which solidified Long Covid as a disease entity

We have known Covid can cause prolonged disease affecting the brain and central nervous system for years. This is including after asymptomatic/pauci-symptomatic and so-called mild acute cases. Many of the first reports came from first-wave survivors themselves

In a sample of 2883 Covid survivors from California 🇺🇸 interviewed at least 30 days post Covid, 20.9% reported "incomplete recovery" and 4.9% "severe" LC. Severe acute Covid and being unvaccinated appeared to be predictors of #LongCovid in the study

The study, thus, assessed "the prevalence of long COVID across five different definitions including, 1) persistent COVID-19 symptoms, 2) incomplete recovery, 3) sought healthcare for long COVID, 4) functional impact on daily activity, and 5) severe long COVID"

In a sample of 205 patients with COPD and SARS-CoV-2 Omicron, 2022, COPD did not independently increase the risk of COVID-19 mortality. However, COPD patients were more likely to require oxygen and noninvasive ventilation than non-COPD

The retrospective cohort study investigated the association between chronic obstructive pulmonary disease (COPD) and the prognosis of COVID-19 patients infected with Omicron variant.
The study found that "COPD itself was not an independent risk factor for COVID-19 mortality. Age, sex, vaccination status, comorbidities, and long-term oxygen therapy use were important predictors of mortality."

Covid isn't the "common cold". Covid=common cold/flu is an argument that some policymakers, pundits, and so-called "experts" have been trying to make since much earlier in the pandemic. They can do it directly, or indirectly. It happened with Omicron, for example. Some examples:
Tony Fauci in late 2021, Omicron, we likely learn to live with Covid like we do we do with common cold and flu (to note, Fauci has expressed varying and in my opinion inconsistent views on Covid, see various media)
