Previously I posted proof that the janitor is James. Now I'll talk about James' past before that time,and the trauma he suffered. This will be a very long post,with proof and a lot of things to back it up, and many hidden details that most of you missed. *SA is mentioned* (1/?)
First off, the x-rays in Brookhaven are James', you can see his jawline and his teeth, especially his crooked canine tooth that sticks out. There is also some explanation for the x-rays in the game files. A big sketch of James' face is also in a corner of Brookhaven's lobby.(2/?)
The remake expands on the novel in which it is confirmed James had been a mental patient in the past. Masahiro Ito also mentioned this last year. His memories and trauma from his hospitalization appear throughout the entire game, as much as his guilt for what he did to Mary.(3/?)
In the SH2 novel James recognizes a note in Brookhaven and says it was written about him a long time ago.For the rest of the story he often wonders if he's still hospitalized and hallucinating and whether his psychiatrist (the director we read about) is leaving him items and(4/?)
he asks the director for help inside his head. In the novel however, the director wasn't implied to be a terrible person.
In the remake Bloober took their own creative liberties and expanded on James' hospitalization, mental illness and trauma A LOT. (5/?)
The "three patients" are all James and the game files confirm they symbolize fear, disgust and aggression, just like the three paths he takes to unlock the box in the hospital otherworld, and the three paths in the labyrinth. There is a reason these emotions (6/?)
...are present so much in the game, and it isn't related to Mary and how he felt about her while she was sick. Also the hand on which we put their bracelets is James' hand model. (7/?)
Note this tombstone that only appears in NG+. This quote is from the movie Tenebre, in which the protagonist becomes a murderer after enduring violent sexual humiliation, and the symbols above are the "aggression" patient and the aggression path in the labyrinth. (8/?)
The exact reason James was hospitalized is unknown, note #1 is the only clue and it doesn't say much. The novel, the remake and note #2 that also appears on the original game imply heavily that he has schizophrenia, and as a track in the sh2r bonus ost is titled "schizoid"(9/?)
In this unused note by an employee,the director began acting strange and became obsessed with James after he arrived.The fact they tried to erase the phrases "he has become obsessed" and "I'm afraid that" implies that he had a history of abuse and staff was afraid of him(10/?)
In an unused note the director says that James reminds him of someone, but he tried to erase what he wrote. This is related to the note we find in the safe in his office. It may appear to be about Mary and Maria but it's the director's handwriting and it's about James. (11/?)
We find many notes throughout the hospital sequence that imply SA, and in some of them it's obvious and undeniable. The first note is this, in a small hole in the wall with the "aggression" patient's symbol painted around it,which turns into a gaping hole later in the game.(12/?)
Another hole with the eye painted around it is later found in the pool wall, (I forgot to screenshot) where this SA attempt took place. It wasn't the first time, as it apparently triggered his ptsd and made him attack the director, then he went hysterical and ran away. (13/?)
This note shows James's behavioural and physical symptoms, note them because they are important, I will explain soon. He also had to be restrained to be bathed. They only discovered something they suspected to be "self-injury" on the day they bathed him, so you can imagine(14/?)
...what kind of injury it must have been. Also the fact he refused to bathe to the point he had to be restrained and forced could have been a form of self defense, trying to be disgusting so that his abuser wouldn't want to touch him.(15/?)
These are the explanations for his x-rays found in the game files (the last one is in the game) . The truth is most likely that he didn't get a digestive tract inflammation by hair pulling and swallowing. All the symptoms and behaviours he shows in the notes are ALL of (16/?)
the signs of Rape Syndrome, as it is scientifically called. Recent SA victims are constantly overwhelmed by aggression,fear and disgust,and many of them have frequent vomiting,digestive tract inflammation,loss of appetite, and staring/touching them may trigger their PTSD. (17/?)
The "girl" in this note is the "fear" patient, and we can see the mental state he was at when he got moved to room L1 f
to be monitored closely (as per the report in the previous post). Being around so many people terrified him so he made a little tent to isolate himself. (18/?)
The director also has a photo of the tent in his office, we find the "fear" bracelet in another tent, and we find the same tent in the "fear" path of the hospital otherworld, with a note that tells something more than just the solution to a puzzle. (19/?)
James' x-rays are supposed to belong to the "disgust" patient, and at this point he had been moved to a single room with a big window from which the director would watch him at all times, as his report I posted previously says. (20/?)
When James lifts the "disgust" patient's blanket he finds a pile of intestines, and it's one of the very few moments in the game that get such a strong reaction out of him.(21/?)
The note on the rooftop is changed in the remake to show that James was determined to do something he had been planning. According to later notes, both in the hospital and prison (which is a continuation of the same story), James stole the night guard's keys (22/?)
and planned to kill the director. It's told in various ways, literally and metaphorically.
He was caught a couple of times. The last note in my previous post is unused and talks about the prisoner who hid the shiv in the isolation cell in the final version of the game. (23/?)
And according to the game files the "thief" in that note is also the same prisoner who wrote this note on the floor of his cell. It's not about taking objects, it is a metaphor for non-consensual sex. Notice the hole in the middle, which also pulsates in the game. (24/?)
"Reap what you sow" is written on the path to the "warden's" office (it's the director's office, it looks the same and the typewriter is there to make it obvious, also the back room is there).James' obsession with getting revenge is the main focus in this part of the game. (25/?)
Most SA victims have obsessive thoughts about revenge, usually as a means to cope with their shame and self-blame.
He eventually succeeded. The murder is hinted in a few different ways, but it's clearly told as two of the innocent prisoners' stories in the gallows puzzle.(26/?)
This hole and the message are in the prison showers, where the "prisoner" supposedly locked himself up with his victim. The bathtubs are also a common theme in the game. I'm not sure if this represents where he killed him, or if it's one of the places where he was SA'd. (27/?)
His weapon was possibly a rusty scissors. The barber shop in the opening has a huge rusty scissors outside, and many more inside, one is next to a note and a straightjacket. (Look at the assets' name). That would also explain why pyramid head's weapon is a half scissors. (28/?)
After that, from the notes it seems James was locked up in an isolation cell. If a person is proven to be too severely mentally ill they can evade prison and instead get treatment in a high-security facility. (29/?)
This is also evident when he enters an isolation cell and gets locked up and trapped for some time and a voice laughs at him, and there's a note inside in which he apologizes to himself and explains he just wanted to protect himself. (30/?)
Abstract daddy looks different to James than to Angela (confirmed) and Mateusz Lenart said the abstract daddy sequence has a personal meaning to James. In the novel James says the reason he saw abstract daddy was because "he hides the same darkness as Angela" (31/?)
Abstract daddy has James' limbs. He's wearing medical mitts on his hands, a common restraint for mental patients who self harm or attack others, his knees are red with bleeding cuts (signs of struggle), and he has the same feet as James' corpse in the fridge. (32/?)
Mary and the corpses in the morgue have different feet types so it was intentional. Looking at his anatomy, it has to be the victim's limbs,not the assaulter's.Also the voice on the TVs is the director's. It may simply be the same actor or it could be intentional. (33/?)
When he first sees abstract daddy it's the only moment in the game when he is truly mortified and about to cry (I used camhack), and nothing else gets that kind of reaction out of him, not other monsters, not killing Eddie, not what he did to Mary or her monster form. (34/?)
The only other time when James is upset is pyramid head's first encounter. Ito said that what pyramid head did was NOT rape but it was meant to look sexual. James maybe believed it's SA, and he looked terrified, although he hardly reacted to anything before. (35/?)
When pyramid head approaches him his eye sockets turn red and he's obviously trying not to cry, and his eyes dart around really fast like a "fight or flight" response even when he's off camera. (36/?)
There are many more representations of SA throughout the game, starting very early into the story. In the apartment in Saul st. (Which was James', see my previous post), we find the first "hole" in the wall. Notice that the broken tiles are bleeding. (37/?)
The room right next to the hole, with the bad smell and the black substance it leaves on James' hand, shows that he was still struggling with his trauma and with substance abuse at that point in his life, after he had been released and had met Mary.(38/?)
The holes in the remake don't represent James' lust,but his trauma. They all look disgusting and violated and James shows discomfort and disgust when he interacts with them. Interacting with a hole and then leaving him idle with his hand near the hole makes his face twitch.(39/?)

@poteissium - them with the feelings of aggression, fear and disgust. There was a lot of focus on "Joseph" who thought he had a daughter, and there was even a puzzle related to a doll he thought was his daughter that the director had given him to get on his good side.
I can't believe I forgot to post this note in which the "patient" clearly describes SA and has extreme hallucinations.

I believe Silent Hill F will focus on self-perception and trauma from psychological abuse. I get the feeling that Hinako is pressured by other people in her life to fit a "format" and meet their expectations, to the point she has lost her sense of self. Explanation below. (1/?)
There's a very frequent theme of dolls in everything we've been shown so far, and at least a few of them resemble Hinako in some way. All the enemies we've been shown are dolls and women mutilated into resembling dolls and puppets. (2/?)

The story of the game is not happening in the present, James has a PTSD episode in the opening according to Luke Roberts. Then he recalls a traumatic time of his life during the first couple of years of his hospitalization,and what led to him staying hospitalized for so long(3/?)
Mary, Maria, Eddie and Laura all have something really disproportional about them. Mary & Maria have very short middle fingers, Eddie has a very short nose and severe underbite,Laura has very long arms.James and Angela are the only characters without some kind of abnormality.Why?
Here is what @draculasbat believes is the meaning behind this. The question is, why don't James and Angela also have a similar kind of flaw? Is there something that makes them different to the other characters?

The room where James finds Laura in the hospital had a much bigger and different role in an earlier version of the story,and it was called "the playpen". The devs have left "bits and pieces" of its original purpose inside,perhaps as an extra layer of storytelling (1/?)
In this early version, the 3 patients' reports were completely different and their stories were mostly the same as the original game's patients (Jack, Joshua and Joseph). James had to collect pieces of their torn reports and piece them together. (2/?)

One of the most disturbing sexual symbolisms in SH2R. When James aims his gun at the "lamp" it reacts and starts moving. After aiming to it for some time the lamp will "masturbate". You can also hear a man's heavy breathing. I don't need to say what the whole ceiling looks like.
It's also important to note that James makes sinister comments on the 2 items in this area,something he doesn't normally do. The mannequin's eye hole drips with the same black fluid as abstract daddy's legs. There's a recording of the director laughing shortly after, on a symbol.
I believe what the ceiling means, is that SA trauma always affects people sexually in various ways (see my pinned post about James).A coping mechanism most common in men is forcing themselves to recall their trauma in some way, followed by feelings of both arousal and self-hatred
