I bet lots of you out there have given patients a slug of magnesium for pesky arrhythmias. I have never really known how it works, but here's a theory.... Image Reactive Oxygen Species (ROS) are a potent source of damage in unstable myocardium. Succinate accumulates during ischaemia and upon reperfusion is oxidised rapidly by succinate dehydrogenase, driving a burst of ROS production. Image

Anyone who has done an ALS course will be familiar with the concept of giving a lipid emulsion such as intralipid to combat local anaesthetic toxicity This all stems from a relatively chance finding in 1998 that the dose of bupivicaine was progressively greater for rats given intralipid infusions in a dose dependent manner.

Troponin, troponinaemia, troponinitis, troponin leaks - distinguishing type 1 MI from type 2 or nonischaemic myocardial injury. Increased demand for a limited supply of evidence... Cardiac isoforms of Tn are expressed in cardiac muscle and released in various fragments on necrosis. Some troponin is unbound in cytoplasm – there is some disagreement over whether these free fragments ‘leak’ across the cell membrane under stress. heart.bmj.com/content/92/7/9…

(picture credit @Medcomic) #HeartFailure #CardioEd Why have we drawn a cut-off of 50% for rEF vs pEF? Well, this relates to older studies which showed that the risk of symptomatic heart failure increased significantly when your EF was less than 50%.
