In this current surge, I've received lots of questions about risk of secondary household transmission from breakthrough infections in fully vaccinated individuals, and specifically about duration of isolation needed. A summary of two recent studies that shed light on this. The first is a study from the pre-Omicron period in South Korea, showing risk of secondary transmission of 7% in vaccinated vs. 26% in unvaccinated. The detection of viable (living) virus was 1.8 days in vaccinated vs. 4.4 days in unvaccinated. jamanetwork.com/journals/jaman…

Importantly, within these groups – viral load was the same in children and adults. jamanetwork.com/journals/jamap… We wanted to see what was happening in the community, not only in hospitals and clinics. There has been a lot of debate around school openings and about whether children in the community could transmit the virus. We thought this study could help address some of these questions.
