In post-4th wave 🇿🇦 interpreting surveillance data is really challenging. In addition to the decoupling of death and cases, we also have decoupling of wastewater viral load and symptomatic cases. In the case of recent peak in wastewater, our cases had been relatively static 1/n As level of prior immunity increases proportion of asymptomatic and mildly symptomatic individual became high. Although these people can still infect others, they have little reason to seek testing and thus not reflected in our case data. 2/n

Our other Western Cape paper of omicron clinical severity pre-print is now out on Medrxiv. medrxiv.org/content/10.110… In this study, we used a PCR proxy marker similar to SGTF in Taqpath assay (approached used in Wolter et al.) to assess risk for hospitalisation among some public sector cases in Western Cape. As the Taqpath assay is rarely used here, we used a different approach. 2/

Casual observation around people I know during 4th wave: re-infection>first infection. This bucks the trend from previous waves. What does this mean? 🧵 1/ Let's get the obvious out of the way: A lot of people had been infected in previous waves. So eventually the trend of re-infection vs first time has to change. Our short pre-print on Saliva swabs are the preferred sample for Omicron detection medrxiv.org/content/10.110… Compared to Delta where saliva samples had substantially higher Cts, there is no difference between Cts of omicron saliva samples and nasal swab. Image
