Today I’m going to cover some basics of asthma. 🫁
Most of this content comes from this year’s GINA report.
Asthma is characterized by chronic airway inflammation, VARIABLE symptoms, and VARIABLE airflow limitation/airway hyperresponsiveness.
It is a clinical diagnosis, not a spirometric one as spiro findings are “not necessary or sufficient to make the diagnosis.”

I recently gave a COPD boot camp lecture and wanted to share some of the high points. 🫁

Much of this content comes from the 2023 GOLD report + the 2005 and 2022 PFT interpretation papers (these are great resources to keep handy).

New Lung-RADS guidelines!
There are quite a few changes to our former Lung-RADS system--let's go through them here.
The first time you do a procedure, lower your expectations for yourself.
Don’t expect to do the whole thing well, or even what is considered to be the main part of it.
Expect to learn some microskills—negative pressure on a syringe, gowning yourself, opening the kit, etc
and then the NEXT time you attempt it, your brain will be familiar with those parts and will be freed up to focus on the next microskills—planting your hand, following a needle tip, making a skin nick, etc.

I have to tell you a story about my sweet 6 year old.
Tonight as I was putting him to bed he didn’t seem like himself.
He is always wide open, always goofy, always grinning, and always talking up a storm.
Tonight he was serious, quiet, and seemed bothered about something.
I asked him what was up, and he said nothing, then he said he was tired, the he said he was hot.
I didn’t buy any of it, but I was alarmed because he NEVER has secrets.

Incredible pictures from the 1970s of the construction of the New River Gorge Bridge near Fayetteville, WV.

It was the world’s longest single span arch bridge for 26 years—now it’s the fifth longest.

Because I'm interested and I want to know everyone else's thoughts, this is going to be a mega thread on the new PFT interpretation guidelines coming from ERS/ATS.
🧵
First off, this isn't technically published yet. It says it's an "early view," but that it's been accepted to the ERJ so I assume it will be out soon.
