This is so true!
During MBBS we were never taught this.
During post grad it was simply trust upon us.
As an ED specialist have seen more than my share.
The ED is probably the worst place to "declare death" as it is called due to the total lack of privacy/quiet in the department. So yes it was learned the hard way. But it shouldn't be like that. While I wish I could say it gets easier, it really doesn't. Every patient's death weighs on us. We really are the "walking wounded" as they term us.

In this🧵I would like to go through what a doctor could do.

60 year old female brought to ER by her very concerned family.
She had gone to the dentist for a check-up when the dentist noticed that there was a "deviation" of one side of the mouth (shout out to that dentist) & suspected she might be having a stroke thus referred her... Image The lovely lady was insistent she "didn't want to be a bother" and that she noticed that for the "past two days" whenever she "drank water a little bit would dribble out down one side of her mouth"
She didn't think much about it at the time till she met with her dentist.

A must read for all secular Indians travelling to Australia

Australia is an expensive country, but its civic services are of the highest standards.
During Christmas, a family from Banaras, UP had gone to Australia for a holiday. It included a couple, their two children and the man’s father.
After three days in Sydney, they hired a car to go to Melbourne. The freeway from Sydney to Melbourne was just superb.

Why is Christmas celebrated on December 25th?

It's that time of the year again when most Christians in the world celebrate the birth of Jesus Christ. I say most because there is a significant number of Christians who celebrate on January 6th & we'll get to that in a bit!

A 🧵 Initially Christmas was celebrated on different dates depending on where in the world one was at the time.

Dates included
January 6th*
January 7th
March 21st
May 20th
May 28th
December 25th*

there is January 6th again!

Hi!

Emergency Doctor here

Small correction to our friend's rant...

1. Meat and fish can take as long as 2 days to fully digest

2. Most vegetables are NOT digested by human beings but instead go straight through the GIT untouched

let me explain point 2 with a short thread 🧵 Your body can't digest or absorb fiber.

Both insoluble (wheat bran, veggies, and whole grains) and soluble (oats, fruits, veggies, and beans) fiber are not digestible.

Russell's teapot is an analogy, formulated by the philosopher Bertrand Russell (1872–1970), to illustrate that the philosophic burden of proof lies upon a person making empirically unfalsifiable claims, rather than shifting the burden of disproof to others. He wrote that if he were to assert, without offering proof, that a teapot, too small to be seen by telescopes, orbits the Sun somewhere in space between the Earth and Mars, he could not expect anyone to believe him solely because his assertion could not be proven wrong.

After seeing so much "outrage" among fellow doctors about this issue, I was wondering what the division percentages were. 🤔 so out of 100 seats
60 are "reserved"
40 are for open category

seems fair for everybody in my view

so what's the problem?

why don't you ask for more colleges to be opened?

why change the ratio?

Some people are on social media just to get a few likes/shares/retweets etc

They will give up their integrity and dignity to say the most vile things

All in the aim of getting the online attention they so desperately crave to substitute their pathetic offline lives For the rest of us who have a lot IRL social media is just an "escape"

A means to take a break maybe connect with some "online buddies"

None of this online stuff should ever be taken seriously. This isn't real life. Real life is out there. Just look above your screen

Unfortunate remnant of our British past
In UK even today the ED is called "Casualty"
The term Casualty in India has negative connotations
As we have been trained with American based books and protocols we are more used to the term Emergency Medicine Department or ED In India historically and in many places to date "Casualty" was staffed by junior inexperienced freshly graduated MBBS doctors who were poorly trained in emergency situations. They were employed with minimal pay and for these doctors it was a chance to gain soke experience.

"There are 3 kinds of truths in our world today.
1. Objective truth
2. Personal truth
3. Political truth "

Neil deGrasse Tyson
American astrophysicist

#FoodForThought 1. Objective truth
This is what is proved by science. Whether you believe it or not it is scientific fact. It has faced numerous testing, experiments & has been proved to be true by multiple people across the world using different methods & materials yet coming to same result.

Rapid sequence intubation (RSI) is an airway management technique that produces inducing immediate unresponsiveness (induction agent) and muscular relaxation (neuromuscular blocking agent) & is the fastest & most effective means of controlling the emergency airway. RSI is particularly useful in the patient with an intact gag reflex, a “full” stomach, and a life threatening injury or illness requiring immediate airway control

Some people also call this as "Drug assisted Intubation"

"The dictate that one 'not speak ill of the dead' is (at best) appropriate for private individuals, not influential public figures.

While the supporters of this person will rightfully feel aggrieved the opponents equally have their right to react how they please." So enough with the moral high horse shithousery
People can say what they want about him
That's their right
And people can abuse each other for it
That's what social media is for isn't it?

Personally it makes no difference to me what happened to him except for one tiny point.

let's get a little more technical with this explanation

so we know that atmospheric air is 21%

but when the lungs are damaged we need to give oxygen support in higher concentrations to overcome the damage and allow oxygenation

we use different delivery devices for this Nasal cannulae
Deliver 24-30% oxygen
Flow rate 1-4L/min (4L will dry the nose, 2L is more comfortable)

As long as the propaganda plays up the "there is no oxygen shortage no bed shortage all lies to defame Modi" lies scenes like this will be the norm

To my fellow HCWs
My humble suggestion
If family members get agressive De-escalate & walk away
In a pandemic your life is priority Learn & keep ready exit strategies
Do not engage with agressive behaviour
Call the police & escape

I repeat again
In a pandemic your life is more important

We are on our own
This government won't help
Their cheerleaders may abuse you online
But not worth your life
Good luck 😞

Please specify which vaccines those bodies are advocating and which aren't?
Maybe that's why Indian guidelines "are still blurred"? Just an example

The American College of Obstetricians and Gynaecologists only mention the 3 vaccines they use

A certain doctor with a moderately decent twitter following is outraged over the "media coverage of the funeral pyres all over the nation" & is demanding to know "why aren't the media focusing on mild/moderate cases & the recovery rate instead"
I honestly have no reply for this🤷‍♂️ This would be a good response 👍

Good Morning
Short answer is NO - utter nonsense
Long answer
This forward has two parts to it
1 true & 1 not
While the role of varying levels of progesterone & oestrogen affecting the immune system has been seen in some studies (true)
Vaccines DON'T DECREASE immunity (false) Vaccine trigger the immune system to produce antibodies by mimicking an infection. That's why many have fever after a vaccine. You are literally being infected by a milder form of the disease to allow your body to have a practice sparring session with it.

Excellent Doubt
As we learned in school, Earth's Atmospheric air is made up of
Nitrogen 78%
Oxygen 21%
and the remaining 1% made up of other gases
See Pie chart below

So we are breathing this air
We need ONLY 21% right?
Why 99.9% oxygen cylinders then?
This answer is a little long so bear with me. I will try and keep this as simple & brief as possible.

To start with 4 questions...
What is ventilation?
What is oxygenation?
What is respiration?
What is the difference?

Let's talk about Oxygen

What Are The Differences Between Medical Oxygen And Industrial Oxygen?

Medical oxygen is high purity oxygen that is used for medical treatments and is developed for use in the human body. Medical oxygen cylinders contain a high purity of oxygen gas no other types of gases are allowed in the cylinder to prevent contamination. There are additional requirements and rules for medical oxygen, including requiring a person to have a prescription to order medical oxygen.
