So my lady friend eats a quite varied diet. Mostly whole foods, but some processed. Lots of veggies, some fruit, a little red meat, often fish, eggs, cheese, poultry, yogurt. She has bacon for breakfast two or so days a week, and I'm talking 4-6 rashers. 1/n She has bread nearly every meal, enjoys pasta, rice, quinoa, beans, potatoes, nuts, seeds, and occasionally ice cream, fudgsicles, and milk (yuck) chocolate. I am fully aware of the discussion on lowering LDL-C, and avoiding statins, PCSK9i, and all cholesterol-lowering drugs. I'm also aware that sky-high LDL-C on a carnivore or VLCD can be mitigated by adding a few high-quality whole food carbs into the diet. No need to rehash that in this thread.

Ten years ago this week, I quit heavily drinking alcohol cold turkey and quit eating carbs. I weighed 256lb (I'm ~5'11'"), waist:hip was 1.16, and waist:height was 0.7, trigs were 393. Many times over the decades (since 1974), I've been right where I am today: At a low, nearly optimal weight after a fairly profound weight loss. Before 2014, I reduced weight via calorie restriction and willpower. It's a miserable process rife with hunger and (unknown to me Image at the time, negative metabolic and physical changes). Looks at lab report — LDL-C 270 (350 last time), LDL-P 2504 (3421) — and recites should be <100 and <1000. Says nothing about HDL 77 or TRIG 69 or LP-IR <25). Freaks out. I can tell this report colors everything else he says, and how he says it. 1/n "Your CAC was abnormal." (17 this time, zero three years ago). Then adds, but anything less 100 is lower risk. "Your carotids are worse this time." Then adds but the % occlusions are not very accurate. Recommends a statin, plant-based diet, avoiding SFA, yada yada. 2/n

This is the 3rd day of 3 high-fat, calorie-doubled days before a lipid panel tomorrow. Curious to see if TC/LDL is lower than the last time, when I was eating my normal LCHF diet. Question about the 3:1 or 2:1 keto diet for tumor suppression

Adequate protein vs excess energy

115 lb IBW (52kg), 122 lb current weight, maintaining on 1400 kCal/day 1.6 g/kg protein target is 84 g/day protein, which means 168 g/day fat at 2:1 ratio
which is 1848 kCal/day

So adequate protein with 2:1 F:P results in excess energy.

Today is the consult with the neurosurgeon. If you have a minute to send good vibes, please do. I haven't been this anxious ever. Verdict: 7+ hour brain surgery -> ICU -> 4+ days in hospital. Pathology will determine exact type and necessity of chemo/radiation. Lots to go wrong, but we can't just leave the thing alone. Thanks everyone for the good vibes and kind words. It really helps.

What I'm doing right this minute and why. It will likely change!

1) Set your protein 1.4-1.6g/kg/d, more if you exercise lots
2) Set your carbs ≤20 if you're sick, ≤60 if you're fat-adapted and metabolically well, maybe more if you exercise lots 1/4 3) The rest fat: tallow, lard, butter; coconut, avocado, olive oil (if from trusted sources)

Eat eggs, cheese, full fat dairy, beef (ruminant); pork & chicken can be high in n6; berries, 100% chocolate; avocado, toasted coconut if you like

You won't die if you stop eating carbs. You won't die if you fast for 18 hours. You won't die if you make changes to you diet to find out what works for you. I get such a kick out of my cardiologist. On my first visit, he saw my lab report and just knew I shouldn't be buying green bananas, my cholesterol was so high, and I don't eat fruits and veg. He remembered I eat keto or Paleo (same thing to him I think). After 1/4 things had settled down, he confirmed we found high HDL, low trigs, low CRP, low insulin and A1c, and a zero CAC, and that he didn't see the need for a statin. Two choices: Follow the recommendations of your doctors, RDs/CDEs, and other HCPs. Eat low-fat, favor seed oils over animal fats, eat 7x a day, lots of heart-healthy whole grains, high fiber, limit meat, no full-fat dairy, etc. You know the drill. Take a 1/10 statin for your LDL-C, BP meds if you're over 120/80, allopurinol for your gout, steroid creams for your fungal infections, inhalers for your asthma, antihistamines for your allergies, metformin and SGLT2i and insulin for your blood sugars, and more pills 2/10
