On 29 Dec 2018, a patient at Hacienda Health Care shocked staff by going into labor. She was 29, severely disabled, and they’d had no idea that she was pregnant. 1/ The victim had been at HHC since 1992, after childhood seizures left her profoundly disabled. She is unable to speak, although she responds to pain by crying and vocalizing her distress, and her mobility is severely limited: she can only move her head and arms weakly. She also requires ventilator support. 2/

This is the story of a murder that was allowed to happen by the state of California, committed by a man who should have been in prison. 1/11 On August 14, Rebecca Haro reported that her 7 month-old son Emmanuel had been kidnapped. According to Haro, she’d been attacked and knocked unconscious while changing her son’s diaper in a store parking lot by someone who said “Hola” just before the assault. 2/11

🧵 This is Jakob “Dakota” Nieves, a well-known trans activist. He was arrested at the age of 19 after sending an undercover federal agent video and photos of himself raping a child under the age of four.

Forensic analysis of his phone produced videos and images of (1/3) of Nieves sexually abusing that child, as well as a second victim who was under the age of four. His phone also contained thousands of images of children being sexually abused, depicting at least 175 separate victims, none of whom were known to him. (2/3)

🧵 This is a look at the US health care system’s guidelines for assigning trans patients to rooms in inpatient mental health facilities.

Fair warning - this is a very long post, but this issue is extremely important. These guidelines directly affect the safety and mental health of all the patients involved, trans or not, and no one is talking about it.

This is also important to me on a personal level; I have a niece who struggles with bipolar disorder and occasionally needs hospitalization, so my usual appreciation for anyone who sticks it out to the end is tenfold today. (1/) It is important to know up front that the vast majority of US mental health facilities provide acute care only. Acute care means that patients are there for a few days to a week in order to be stabilized and then released. Many are there involuntarily, having been judged by a magistrate to be a clear and present physical threat to themselves or others.

Although units are almost always mixed-sex, rooms have always been single-sex, and while some facilities do put their most disturbed patients in a more intensively controlled ward, this is not by any means a universal practice, and no attempt is ever made to separate the involuntary and voluntary patients, or to separate patients with a criminal history - if that history is even known.

This means that a self-admitted suicidal 18 year old girl can end up in the same unit as a grown man with a history of sexual violence and any psychiatric condition you care to name whom a magistrate has declared is temporarily too dangerous to be out on the street.

Violence is not at all uncommon in these facilities; nor is inappropriate sexual contact between patients, with much of it being exploitative, and unwanted by the (usually young and female) target.

Facilities are often badly understaffed, leaving the employees they do have overstretched, which means that there is almost never any adequate supervision of interactions between patients during free time.

The number of women in these facilities who have been the victims of male violence is exceptionally high, and this trauma is often a - if not the - major contributing factor to their need for acute psychiatric intervention.

This, then, is one of the environments for which the following guidelines have been written. (2/)

(1/) is In 2006, a 12 year old girl in the UK was kidnapped and raped by 2 Muslim men before being thrown out of their moving car.

She begged a man for help. He agreed, and invited her in to clean up, only to sexually assault her himself. She escaped as he was phoning his friends to come join in.

As she made her way along the street after her escape, a passing taxi driver and his passenger stopped to ask if she needed help, and offered her a ride to the police station. Instead, they took her to a house, forced her inside, then locked her in a room where, for the next 24 hours, she was raped over and over again by 5 Muslim men.

The orchestrator of this last atrocity, one Shakil Choudhury, got a grand total of 3 years in prison.

In the space of just a few hours, this girl encountered two groups of Muslim men, all of whom raped her, and another man who assaulted her and was only prevented from sharing her with his friends by her escape. That’s 3 groups of gang rapists in one British town, all the way back in 2006; god only knows how many others are out there now — and the cops and the courts couldn’t care less if they tried.
