@rose_m_olson This case highlights (at least) two things:

1. The importance of disease prevalence
2. The benefits (and drawbacks) of the Problem Representation
@rose_m_olson When I wrote the Problem Representation, I included Egypt. This biased the differential diagnosis.

Had the diagnosis been leishmaniasis (MY final diagnosis), this would have appeared prescient.

But the actual final diagnosis (sarcoidosis) makes its inclusion appear irrelevant.

Sarcoidosis: 25,000 new cases/year US
Visceral leishmaniasis: 0 new cases/year in US

[Also, note that Egypt had no cases in 2018.]
