DIC vs Liver disease...

It's hard to distinguish between these; both cause
1. Coagulopathy
2. Thrombocytopenia
3. Liver enzyme elevation (😉😜 @medrants @DxRxEdu )
4. Cholestasis
5. Hypoalbuminemia

If we REALLY need to know (often we don't), send of a factor 8 level...
In DIC, all factors are consumed, including factor 8.

Why are we singling out factor 8?

It's made in the endothelium, and therefore, will be normal/high in liver disease.

It's a great test, but takes a while to come back.

DIC, like many critical illnesses results in a metabolic acidosis - lactate acidosis, ketosis, and kidney injury - all are potential contributors.

In liver disease, respiratory alkalosis is a common finding, and is attributed to the accumulation of progesterone!
