So many great tutorials lately, I’ll contribute this review on some prostate cancer basics.
Prostate cancer 101

2. Slightly higher power, ill address each of the circled areas.
3. Orange box: Atrophy. Make a mental note of the lobulated architecture (red circles), a benign feature.
4. Same area. Say the biopsy is roughly the red rectangle (left), cropped out at the right. Glands now look more haphazard; sometimes it can be hard to see the lobulated architecture of atrophy on needs bx
5. Some features of atrophy at higher power, which can be helpful if the lobulated architecture isn’t apparent.
6. Area number 3 (red circle): This is probably some high grade PIN. Low power: darker than adjacent benign glands. Higher power at right, with annotation
7. Area # 2 (yellow circle): this is probably some low grade cancer. Note how it weaves in between the adjacent glands, in contrast to the lobulated architecture of atrophy

9. Say this is the biopsy, red rectangle. Having seen the section, we know that the top is probably HGPIN and the bottoms probably cancer. However on a biopsy it can be tough to tell “outpouchings” of PIN from ca. Sometimes “HGPIN w adjacent atypical glands” is appropriate
10. Also sometimes the biopsy only catches the edges of a cancer. In these cases, “Small focus atyical glands”/“ASAP”/“X atypical glands” can be appropriate
11. Tip: look at the high power features of these areas, then go back and look at the same areas at low power. This will help you recognize them at low power.
