Mayer ties the use of "stop and proceed" in #wmata to 2009 and the change from ATO mode to manual mode. It's easier in manual mode for operators to move trains with Zero Speed Commands than in ATO.

The operator of the train which ran into the back of another last October was suspended three times, but was still working at the agency.

* 2011: 12 days, red signal violation
* 2015: 3 days, failure to report a station overrun
* 2015: 3 days, talking w/cab door open

Important to note that these safety investigation reports are written by the #wmata safety department, given to WMSC for review. If Metro doesn't call out an issue in its initial report, it's possible WMSC won't know to have it included.
