How should a family of 4 who live in a 2BHK apartment handle different scenarios of Covid infection?
How should they quarantine if 1, 2, 3 or all of them test positive?

Scenario 1: ALL four in the family test positive.

✅ No need for separate quarantine.
✅ Advised to wear double mask/N95 when together.
✅ Do not venture out.
✅ Limit items used inside the home to minimize sanitization later.
✅ Take rest and medicines as advised.

Scenario 2: One person tests positive.

✅ Isolate the person in bedroom with attached bathroom.
✅ Others to share other rooms.
✅ Others to wear mask, sanitise objects touched and keep distance as much. They could turn +ve in future due to contact.

✅ If others develop any symptom: Isolate immediately and test. Otherwise advised to test 5 days after contact with Covid +ve person.
✅ Covid +ve person to use own set of utensils and not to come out of room. Avoid all types of contact with others.

Scenario 3: Two out of 4 persons test positive.

✅ Let the 2 positive persons share the bedroom with attached bathroom.
✅ Others to share other rooms.
✅ All precautions mentioned in scenario 2 holds good.
✅ Try to move any sr. citizen who is not +ve out of home.

Scenario 4: 3 out of 4 persons test positive.

✅ Best approach would be to try to move one or two of the positive persons to a Covid care centre.
✅ If not possible, one option is to isolate the person who is NOT +ve in the bedroom with attached bathroom.

✅ The other option is for the 3 Covid +ve persons to be isolated in both the bedrooms. The 4th individual to sleep in the living room and share bathroom with one Covid +ve person.

📌 In reality, these scenarios might happen sequentially. Plan for all.

Handling homes with children:

✅ Parents test positive and children negative:
📌 If possible, isolate kids from parents. Move kids into homes of friends/family if possible.
📌 In case of infants/kids who cannot be separated from parents, no option but to stay together.

📌 Parents to ensure that they wear mask and sanitise hands when around children.

✅ Breastfeeding mother:
📌 It is fine to breastfeed babies.
📌 Mothers to wear double mask and sanitise hands before feeding. Try to limit time of feeding. Clean chest if mother was coughing.

📌 Tips on using a shared bathroom:
✅ Have an order for using the bathroom, with the Covid +ve person using it last.
✅ Keep exhaust fan on all time. Open ventilators if possible.
✅ Do not share toiletries and towels. Do not keep them in bathroom.

✅ If health permits, Covid +ve person to disinfect bathroom after use. To definitely sanitise door knob, taps etc. after each use.
✅ Wear a mask even inside the bathroom when possible.
✅ Leave a gap of 45 minutes to 1 hour between use.

Recommended gap between 2 doses of #covishield increased to 12 to 16 weeks.

📌 A look at the trial data behind this and practice followed by other countries.

✅ Key finding from AstraZeneca (Covishield) studies that showed an efficacy of 76% for 90 days after single dose.

✅ No significant reduction in protection seen from days 22 to 90 after a single dose - implying that even a single dose can offer significant protection for upto 3 months.
✅ In this case, it was only prudent to spread out the doses so that more could get atleast 1 dose.

✅ Additional data that showed increased efficacy (for symptomatic cases) when the gap between 2 doses was > 12 weeks. Efficacy of 81% noted with a > 12 week interval.
✅ These 2 factors have lead most countries administering Covishield to recommend a gap of 8 to 12 weeks.

While most will recover from Covid from home, there are 2 things that set this illness apart:

Isolation and Uncertainty.

📌 Weariness of Isolation: 14 to 17 days of being alone in a room can take its toll. It can feel monotonous, restricted and lonely.

While a Covid positive person goes through this individual struggle, those taking care of them are fighting a difficult battle as well. They have to take care of everything from food, medicines, arranging essentials, waste management and more; all while trying to stay safe.

Imagine a home where multiple persons have tested positive or with children. The caretaker has a tough task managing regular chores and Covid care for a 2 week period. Even a small task of giving a glass of juice is complicated!

North Eastern states: Vaccination coverage by state and district.

✅ Tripura: 11.25% of total population has received 2 doses (2nd behind Ladakh) and 24% have received atleast 1 dose.
✅ Assam: Lagging with close to 2% with 2 doses and 7% with 1 dose.

✅ Arunachal Pradesh: 4.5% received 2 doses and 14% received 1 dose of the vaccine. 3 districts have > 10% receiving 2 doses.

✅ Manipur: Only 2.5% received both doses and 8.5% one dose. No district has >5% coverage of 2 doses.

✅ Meghalaya: 8% have received 1st dose and 2% both doses. All districts showing <3% coverage for both doses.

Variant B.1.617 (or incorrectly called double mutant in popular jargon) is one of the most dominant variants in India now.

What does it mean for you and me?
📌 We have (have had for many months) a more transmissible variant amongst us.

📌 The tools we have to protect ourselves remain the same irrespective of the variant: Masks, Social Distancing, Hand hygiene, Ventilation.
📌 With a more virulent variant, we need to be more stringent about enforcing these safety norms at an individual and community level.

➡️ Use double masks or N95s. Use of a single cloth mask should be avoided.

Isolating in a home/hostel with no separate room or bathroom:

📌 Most recommendations on isolation assume that a separate room/bathroom is available. This is not always the case - many live in shared spaces.

In this thread we look at ways to isolate in a shared room.

✅ Separate bedroom: If a separate bedroom is available, isolate the Covid +ve person there. Other residents have to temporarily use other rooms in the house, even if it means sleeping in the living/dining rooms. As much as possible do not share bedrooms.

✅ No separate bathrooms: If the bathroom has to be shared with a Covid +ve person, here are some tips to minimize risk:
📌 Have an order for using the bathroom: Let the Covid +ve person use the bathroom last, after others.
📌 Sanitize bathroom after each use.
