This is why FSD has to be near perfect or not exist. There is no middle ground

@Tweetermeyer when people like myself who do process safety, we can never look at software or safety systems in a vacuum. We have to know how people will interface with them.

@Tweetermeyer A Process Hazard Analysis would look at "banal" things such as: How much overtime are we putting on employees? Are they sleep deprived? Do they have too many conflicting tasks? Is the button needed to press for this critical procedure in a place that it can be located?

@Tweetermeyer This is specific knowledge (they did not have enough training in handling abnormal events) as well as general unawareness of their surroundings.

They relied too much on safety systems!
