A quick run through a simple symptom analysis tool. OPQRST'UV'
Onset - When did it start? How long does it last? How often does it occur?
Provoking & Palliating features - What brings it on? What makes it better? What makes it worse?
Quality - What does it feel like? Can you describe it?
Region & Radiation - Where is it? Does it spread anywhere?
Severity - What is the intensity (e.g. 0-10)? Right now? At best? At worst? On average? How bothered are you by this symptom? Are there any other symptom(s) that accompany this one?
Treatment - What treatments have you tried? What medications have you tried? How effective are these? Do you have any side effects from these treatments or medications? What treatments and/or medications have you tried in the past?
Adding the 'UV' to the original OPQRST mnemonic starts to build in the ICE (Ideas, Concerns & Expectations) elements without being so direct/blunt
Understanding & Impact on you - What do you believe is causing this symptom? How is this symptom affecting you and/or your family/loved ones? What in particular is concerning you about this symptom?
Values - What is your goal for this symptom? What is your comfort goal or acceptable level for this symptom? Are there any other views or feelings about this symptom that are important to you or your family?

The eye (globe) is attached to the orbit by 4 rectus & 2 oblique muscles that collectively allow rotary movement and vision in many directions. These muscles are innervated by 3 cranial nerves (Thread) Image
