In this thread, I’ll be sharing what I feel are the Top 15 most important things to consider when formulating your rank order list (ROL) of residency programs.

So without further ado, let’s get started!

In no particular order:

Resident Vibe

Virtual meet-and-greets are different than in-person socials, but you should have been able to get a general sense of the vibe of the residents from each program. What are the kind of people you want to be around? How did the residents make you feel?

This is arguably the most important factor to consider. These are the people you will be spending the vast majority of your waking hours with on a near daily basis. You want to go to a program where the culture and personalities of the residents resonate with you.

Your co-residents will make work enjoyable, long days bearable, lift you up during tough times, lend a helping hand when you need it, and lead to friendships that last a lifetime. Regardless of where you train, you will always remember the people you shared that journey with.

Location

What part of the country do you want to move to? Do you want to be in an urban, suburban, or rural area? Prefer the warmth or the seasons? Does proximity to family matter? If you want to explore a new part of the country, residency can be a great time to do that.

Cost of Living

Most places are affordable on a resident salary, but this can be difficult in more expensive cities and areas. Create a rough monthly budget. Research types of housing and prices in advance. Think about how far you are willing to be from the hospital.

Hobbies

Don’t forget those hobbies you talked about so much! Residency can be exhausting, and you need to be able to do the things that give you joy and allow you to recharge. Your hobbies will give you the energy to thrive and be yourself. Make sure they are accessible!

Exercise

Beyond the physical benefits it provides, exercise enables motivational goals, encourages a healthy lifestyle, decreases stress, elevates your mood, improves your sleep, and provides opportunities to join communities.

If you like running, are there good trails? If you enjoy team sports, are there local leagues or communities? If you enjoy group fitness like Orangetheory or CrossFit, are there gyms close by, and do the class times/hours work for your schedule?

Career Interests

Are your interests and goals supported at the program? And not only subspecialties, but also other focuses, such as research, global health, and medical education. Have other current/past residents successfully gone on to do what you are hoping to pursue?

PDs and APDs

What impression did you get from program leadership? Looking back, my top 5 ranks were my 5 favorite PDs, and I know that’s not a coincidence. Their passion, attitude, vision, and personality trickle down to the entire program and impact the program culture.

Training Structure

Are you at an academic or community institution? How many weeks long are rotations? Does X+Y appeal to you, and if so, does the program have it? Depending on your specialty, you might consider how many shifts you work or what your surgical load is like.

Autonomy

Autonomy fosters confidence, encourages ownership of your patients, develops independence, provides valuable experiences with critical decision making, and promotes leadership and teaching. The skills you acquire through autonomy translate to your own life as well.

Call Schedule

You don’t realize how much the call schedule matters until you experience it. Is there 24-hour call? If so, which years and rotations? Is there Q3/Qweek (or other) call? Do you do night team? Is there short/long call to get you out early? Cross coverage?

Jeopardy System

Life happens. How does jeopardy work at your program? Who covers jeopardy? Is the jeopardy system a payback or no-payback system? What is the culture of jeopardy like, and how is it perceived by current residents? How much is the jeopardy system used?

Fellowship Matches

Think you may be interested in fellowship? What subspecialties have residents matched into previously? Where have they gone? How strong of a foundation does the residency program provide, and how well-prepared did previous residents feel?

Family

Take time to talk things through with your significant other. Both of your careers and opportunities matter. If you have children, prioritize them too. You want to be someplace that allows your entire family to be happy and supports a healthy and happy relationship.

Response to COVID

Enough time has passed that you should be able to gauge how programs and institutions have responded to COVID and supported residents specifically. How is COVID and vaccination perceived? What are healthcare workers' attitudes and perceptions there?

Your Happiness

You can go to the #1 ranked program and be surrounded by famous physicians and researchers at a world-renowned institution. None of that matters if you’re unhappy. Nothing matters more than your happiness. All of the above contribute to that. Remember that.

A letter of intent is a letter that is sent to the program you plan to rank #1 on your rank order list with the purpose of making that known to the program. Simple, right? There are some things you should know though.

Tip #1: Be thoughtful of your background.

Plain backgrounds are a fairly common choice. Pick a color that allows you to stand out (and not blend into the wall). I don’t recommend virtual backgrounds because of the associated lag and cutting out parts of your body with motion.
You can also have things in the background, but be prepared to talk about them. I had artwork with a favorite quote and a medal from my first half marathon behind me, and both came up in conversation. The rest of the wall behind me was light-colored and blank.

Here’s the thing about out-of-the-box questions. You’re unlikely to get asked them, so I wouldn’t invest much time into preparing for them. However, you may get asked one or two, so it’s not a bad idea to have at least thought about them briefly so you have an answer.
1. What would you give a Ted Talk on?

Quite honestly, this might be one of my favorite interview questions, not because I enjoy answering it, but because your answer tells a lot about your values and passions, as well as what you might want your impact and legacy to be.

Think about the types of questions you ask. A lot of generic information about programs and curricula can be found on program websites or databases. I would encourage you to ask thoughtful questions about things you wouldn’t be able to find the answer to online.
Here are 20 examples of questions I had prepared for all of my interviews. I obviously didn’t ask them all in each interview. I would tend to ask questions based on who I was speaking with and what I still needed to get a better sense of about the program.

1. Tell me about a negative interaction you had with an attending or resident. How did you deal with it?

This isn’t a question to see if you have tough skin, but rather to see how you handle conflict or stress, as well as navigate interpersonal/interprofessional relationships.
2. Give me an example of a time when you had a difficult communication problem.

Good communication is the cornerstone of any relationship, including those with colleagues, staff, and patients. It’s important to recognize when communication is poor and take action to improve it.
