@BradyDale311 was kind enough to send over a journal article that just hit the news today and it appears to offer a great deal of promise. aidsmap.com/news/oct-2022/…
@BradyDale311 As someone intimately familiar with the concepts of reservoir seeding, estimated date of seroconversion, and one who has observed haunting parallels between the pathophysiology of chronic HIV and SARS-Cov-2 infections, I knew repurposing was necessary and have written about it.
@BradyDale311 Molecular docking studies have proven that a number of first line HIV antivirals proved the ability to do so. While there have been studies investigating the use of Truvada as PreP and treatment, the results have been mixed.
@BradyDale311 One would expect the results to be mixed when we have no viral load test to confirm a drop in log copies for those instances where it is used in treatment. How can you say it is not effective, lmao.
@BradyDale311 I had suggested in one Tweet that it made sense to look at the rates of SARS-Cov-2 infection among a population that is made up of varying degrees of immune competence and that is exactly what this study did.
@BradyDale311 For those individuals on a Tenofovir Disoproxil Fumarate/Emtricitabine regimen, Stribild, only 28 individuals contracted SARS-Cov-2. Only 5 individuals who were on the same regimen required hospitalization.
@BradyDale311 For those on a Tenofovir Alafenamide Fumarate/ Emtricitabine regimen, which could either be Genvoya or Biktarvy, 621 individuals contracted SARS-Cov-2 and 156 were hospitalized.
@BradyDale311 Why the difference in TDF vs. TAF? Less TAF is required to exert the same degree of virologic control and is less toxic to the body.
@BradyDale311 So, now we can start to investigate further. Is Elvitegravir or Bictegravir efficacious? What would be the results if cobicistat was added as a separate pill.
@BradyDale311 My hypothesis as to why we haven't seen favorable results with PreP in the past is due to the possibility of Nasal Associated Lymphoid Tissue concentrations not being great enough as that of the Gut Associated Lymphoid Tissues.
@BradyDale311 While the length of suppression is not mentioned, I do know that it takes 6 months of having 200 copies of HIV in your semen before deemed unable to transmit.
@BradyDale311 If you are an individual suffering from a chronic SARS-Cov-2 infection and would like for me to discuss possible antiviral strategies based upon these findings, please reach out.

@EricTopol @TheLancetRheum @LCalabreseDO @CCalabreseDO APOLOGIZE, for history has repeated itself. And here is a 🧵 as to why it is long overdue.
@EricTopol @TheLancetRheum @LCalabreseDO @CCalabreseDO You see, this isn't the first time that our country has seen unspecified pneumonia, lymphocytopenia, viral persistence. We have traveled back in time forty years.
@EricTopol @TheLancetRheum @LCalabreseDO @CCalabreseDO There was no excuse, in this country and in this time, for the spread of a deadly new epidemic.

Should I tell them the band hasn't left the stage and has been playing for 40 years?
There was no excuse, in this country and in this time, for the spread of a deadly new epidemic.
For this was a time in which the United States boasted the world’s most sophisticated medicine and the world’s most extensive public health system, geared to eliminate such pestilence from our national life.

It is grossly irresponsible for anyone to suggest that therapies will be developed for the cognitive decline seen with a SARS-Cov-2 infection. Why? Because they have yet to be created for HIV Associated Neurocognitive Decline.
What do you mean, there isn't a therapy available for HIV Associated Neurocognitive Decline? Why don't close to 1.2 million people living in the United States with HIV not have cognitive impairment and are able to live a completely normal life? Rapid Antiviral Initiation.
That's Right. The medical community has turned a blind eye to one of the most well studied viruses, that had they had the willingness to draw parallels, things would be completely different.

As I am preparing to engage with over 300 individuals who are like those who found themselves suffering during the early days of the HIV/AIDS epidemic, willing to walk a 1,000 miles to slip their skin, I wanted to write a quick tweet based upon an interaction last night.
In HIV management there is a term that is referred to as one's Estimated Date of Seroconversion. This is when the individual went through abject hell, rolling around in a bed that felt as if it were a swimming pool, as they sweat their ass off.
Despite this godawful experience, if an individual is introduced into care within one year of that date, the outlook for living a normal life is more than favorable.

@danaparish I am writing with information that cannot be ignored. It must be discussed and done so frequently because millions of lives depend upon the message being heard at the highest levels of government and the medical community.
@danaparish SARS-Cov-2 uses the ACE-2 receptor to infect the host. There are a number of organs that are rich in ACE-2 receptors. The two most important when discussing an antiviral regimen are the kidneys and liver, with the liver metabolizing the drug and kidneys eliminating it.
@danaparish There was a study in which kidney function was measured in a very large number of individuals and the results were quite disturbing. jasn.asnjournals.org/content/32/11/…

I have repeatedly said that a chronic SARS-Cov-2 infection is deserving of the same treatment protocol as that of a chronic HIV infection. Hopefully, this thread will finally result in that happening. While looking for Bugs Bunny in a damn rabbit hole, people are suffering.
I had read of at least one study suggesting that CD 4 T cells were being actively killed off by a process that did not require the use of ACE-2. Today, thanks to @plantwavesfilms, the last ingredient in a recipe for disaster was provided to me.
@plantwavesfilms The following article, SARS-CoV-2 Uses CD4 to Infect T Helper Lymphocytes, was provided to me. You can read it here. medrxiv.org/content/10.110…
