Monoclonal Gammopathy of Undetermined Significance (MGUS).

MGUS shows up on a serum protein electrophoresis (PEL) as a spike. See figure. The spike is called an M spike.

It can be typed on immunofixation (IFE) to determine if it’s IgG or IgM or IgA; kappa or lambda. See below.

MGUS was a term coined by Dr. Kyle.

He called It MGUS because prior to that it was called benign monoclonal gammopathy. Since he found a 1% per year risk of progression to myeloma he felt it the word “benign” may not capture the clinical implications adequately.

Both triplets VRd and DRd are excellent and well tolerated. Both have beaten lenalidomide/dexamethasone (Rd) in randomized trials.

One adds bortezomib to Rd —-> (VRd regimen)

We know COVID vaccine efficacy wanes over time. Even though protection is excellent against severe illness, you really don’t want to get COVID multiple times.

We know that new variants keep coming up. This bivalent booster targets omicron variants. Get the new booster. Please.
Really inexplicable why there are restrictions on why only those who have completed original series can get the bivalent vaccine.

Even I don’t know how people who haven’t completed the primary vaccine series get the original series or the new booster. Muddled messaging indeed.

After seeing vorinostat, panobinostat, elotuzumab, belantamab, venetoclax, melflufen, selinexor, ixazomib falter in myeloma, my thoughts on cancer drug development for investigators and Pharma. Simple. But simple is hard.

Make sure as a company or investigator you are convinced of the single agent efficacy of your drug in phase II trials. If a drug is weak, needs steroids or something else to take it over the hump, even if it is approved, its likely to be a marginal drug that’s marginally used.
2/
Make sure the dosing schedule is realistic. If a drug is so weak that you think you have to give the most frequent dosing schedule possible so that you won’t miss efficacy benchmark on Phase III, the drug will not fare well in real life.
