3/We tested the effects of physicians prioritizing discharging patients first compared to usual rounding style
☑️3 hospitals
☑️Daily surveys of physicians asking about rounding styles, team composition, and #workload
☑️Intention-to-treat analysis
☑️Outcome: DC order time, LOS
4/ What did we find?
☑️In primary analyses, there was no significant difference in discharge order time, discharge time, or length of stay (LOS).
☑️In secondary analyses that included per protocol and per reported, earlier dc order time but LOS not significantly different.
5/ Qualitative key point: Without also prioritizing other factors, focusing on discharging patients first can lead to inefficiencies.
6/ Take aways
☑️Throughput (i.e., length of stay) was not improved in any analysis.
☑️Findings suggest it may be more effective to support clinicians to use clinical judgment to set priorities. This may include prioritizing dc's first or seeing sick patients first, etc.
2/ With patients in need & hospital halls full,
They sought a solution to relieve the pull.
Early discharges they thought might do the trick,
But what they found, they couldn't quite predict.
With a trial conducted both big & bright,
The results they found came in the night
3/ Did it ease the strain and reduce the pain?
Or did it bring more problems to remain?
The outcome's unknown, the suspense still high,
What the study found, no one can deny,
Did it bring success or lead to strife?
Only time will tell and shed the light.
