3/4: So she went to buy a used folding #bike for Alexey. He took it with him on the plane to Helsinki, and there, Alexey & his bike boarded the train to Imatra. Finally, it was time to put the bike to good use. Because what would have been impossible by foot, was allowed by bike.

#wildlife Rescue. When to intervene & when to leave alone is a question I am often asked. The following 2 videos demonstrates what happened when a dog walker found a baby stoat in quite a bad way, alone, dehydrated & exposed to predators. Teamwork got the kit the help he needed
What happened next? Well the dog walker took that video, showed the video to her neighbour. Neighbour rang & forwarded it to me. I advised him that the stoat kit was in a bad way & needed help. The kit was rescued & placed on a heat pad & you can now see the difference #teamwork
For anyone finding a baby stoat or weasel alone,the following advice may help, but always giving mum the chance to retrieve her kit first as she may be in the process of moving her family. Baby stoat is now safe with @RobertEFuller who has a wealth of experience with this species

After a week of working in the #ICU here in #Seattle caring for patients with #COVID19 (and other illnesses), here are some thoughts. #medtwitter (1/10)
We try to have a high index of suspicion for #SARSCoV2 and test critically-ill patients broadly. However, this means that many patients we test are actually negative. This presents a challenge for clinical reasoning. (2/10)
It takes more energy than usual to avoid anchoring too early and to keep a broad differential.

This is complicated further by the limits of rigorous #Isolation and attempts to preserve #PPE. (3/10)

Thread on learning points from our Cat1 CS #COVID19_Sim @OUHospitals today. Phenomenal engagement from across so many departments: Anaesthetics, Obstetrics, Neonatology, Midwifery, Threatre, Scrub, Infection Control, Portering, Domestics & @OxSTaRCentre. This was crucial 💫
#PPI We had an actor for the first part of the simulation. This allowed us to capture the woman’s perspective. Cat 1 CS is stressful- as reported by our actor. The Obs & MW team were amazing at continuously communicating and reassuring where appropriate despite muffled mask.
#PPE: it needs repeated practice. 1) Be aware of opening doors and contaminating two sides of the door -it is very easy. 2) Try not to wipe your nose/face on your shoulder. 3) How to make donning efficient?(reciprocal buddy?)

2/ This was, as I said yesterday, a challenging operation in difficult circumstances. The area we needed to search was large, very busy & it was after dark. Colleagues from all over @ThamesVP travelled to assist us in #NewportPagnell
3/ The #Community response was immense; so many members of the public offering help, searching, sharing our appeal. Someone even bought coffee for those out in the cold & dark. All of it helped.

Recently our team had to manage a patient who presented critically unwell from post-tonsillectomy hemorrhage - something you may not see that often but can be a true life-threat!

Tip 1: Call for help early!

This patient did well because of excellent #teamwork from scene to OR. We received an alert from the EMS team, and informed ENT early (not in house) to the situation and need for OR. Anesthesia and OR team were also informed to prepare for this

Tip 2: Keep the patient sitting upright

Position is key. This patient was awake & alert, and keeping them upright allowed them to spit out the blood. Give them the suction to use also.

Our new paper is on biorxiv! "Predicting master transcription factors from pan-cancer expression data" biorxiv.org/content/10.110…. We set out to find master TFs in ovarian cancer, and went on quite a journey... A few highlights in the thread 1/
Prioritizing candidate MTFs without ChIP-seq data is tough, so we developed a gene expression-based approach which we loving called the Cancer Core Transcription factor Specificity (CaCTS) algorithm 🌵. (We then decorated the lab in cactuses) /2
Using CaCTS we identified candidate MTFs for 34 major tumor types and 140 molecular/histologic subtypes.

Candidate MTFs tend to be associated with super-enhancers, and many are lineage-specific essential genes - woo CaCTS works! 3/
