Wow. This is huge. This is the real @LillyPad

Eli Lilly cuts insulin price to $35. An issue many of us have been advocating so hard for. #insulin4all nbcnews.com/news/amp/rcna7…
The cap automatically applies to patients on private insurance.

The Inflation Reduction Already capped out of pocket cost to $35 for Medicare Part D recipients.

Huge: People without insurance will pay $35 if they sign up for Eli Lilly’s copay assistance program.
I hope all other insulin manufacturers especially @SanofiUS and @novonordiskus do the same.

This is just in time for @PayorDieFilm premiering March 11 and will shine a light on the huge amount of suffering patients have endured for years due to the cost of insulin.
The caps start later this year after Oct 1.

But most importantly @LillyPad will cut the list price of the non branded “authorized generic of Humalog” fast acting insulin lispro to $25 a vial starting May 1.

I’m particularly grateful that starting April 1, @LillyPad will launch Rezvoglar, a biosimilar to @SanofiUS Lantus at almost 80% discount to Lantus. Interchangeable with Lantus, which means a pharmacist can substitute the drug without a new prescription. usatoday.com/story/news/hea…
Here is a link to the @LillyPad Press Release with all the details. investor.lilly.com/news-releases/…
So what’s next on my wish list and advocacy?

Cost of cancer drugs. Need the same. Affordable meds.
I said $35 not $25 as noted in press release because although the Humalog biosimilar list price was cut to $25 a vial, the out of pocket cap for those taking any other Lilly product will be $35 investor.lilly.com/news-releases/…

That’l will be the max price for uninsured.
Long acting biosimilar list price ~$18 per pen starting Apr 1

Great. And it will be interchangeable.

Why is hard to do anything about prescription drug prices?

It’s like a car with multiple broken parts: engine, battery, tires, transmission, seats, doors, and more.

You can’t fix just one. But fixing all at the same time is also difficult. #MedTwitter nature.com/articles/s4140…
You try to fix one, they point to the other broken parts and say it’s not going to work because so much else is broken.

You can’t fix it all because the opposition to change will naturally now come from all quarters.

It’s just very difficult. And needs enormous will.
If insulin can be expensive then you know it can happen to any drug old or new. If we understand why insulin is expensive you also understand many of the factors involved.

So what can be done? @MayoProceedings mayoclinicproceedings.org/article/S0025-…

Updated mSMART recommendations for treatment of myeloma posted on mSMART.org #MedTwitter

1/ New for relapsed myeloma, clear recommendations based on type of Triple-Class refractory. We define 3 types. @Rfonsi1 @myelomaMD @MorieGertz
2/ New for Relapsed Myeloma - Approach to first relapse based on refractoriness to Len and Anti-CD38.
3/ We feel that bispecific and CAR-T approvals be based on refractoriness to specific drugs and drug classes and not be restricted by lines of therapy because myeloma can be refractory to all major drugs even within 2 lines.

I hope we are moving away from “maximum tolerated dose” in oncology. #MedTwitter

Of the studies I’ve led, the one I am most proud of is the trial that showed that LOWER dose of steroids led to better survival & lower toxicity. @TheLancetOncol
#Myeloma thelancet.com/journals/lanon…
Maximum tolerated dose was an idea used with conventional cytotoxic agents. It’s time has passed.

We should now strive for minimum effective dose or optimal effective dose.
Even with targeted therapies and biologic therapies we have found years after drug approval that the original approved dose was too high and caused unnecessary toxicity.

I am delighted that @PayorDieFilm will make its worldwide premiere @sxsw Film Festival! March 11.

This powerful documentary highlights the plight of Americans with diabetes dealing with the high cost of insulin. #Insulin4All @NSmithholt12 @SarahKSilverman
#PayorDie #SXSW2023
Insulin cost in America shines a light on how broken our healthcare and prescription drug pricing is.

If insulin can be priced high, then anything in healthcare can be priced high. Fixing this needs a lot more than band aids.

Tomorrow we honor and recognize @MorieGertz @MayoClinic with the Robert A. Kyle Lifetime Achievement Award in hematology and oncology.

mayoclinic.pure.elsevier.com/en/persons/mor…
The Award has been given since 2010 at the Mayo Clinic.

Prior winners:
Incidentally Dr. Kyle is probably the only one who has 3 lifetime achievement awards named after a him! One by @IMFmyeloma , one by @WMIWMF and one at Mayo Clinic.

Ayalew Tefferi deserves the credit for instituting the Mayo award.

Now that Twitter shows us the number of “views” for each tweet it’s interesting that:

1) For every 1000 followers you are lucky if your tweet is viewed by 10-100 people.
2) For corporate /organizational accounts views relative to follower count seems quite low.

A million followers doesn’t mean million people see the tweet. Far from it! More like a few thousand views on average.
3) Individual accounts seem do better but still lower than what the follower count would suggest.
