An otherwise healthy 8 yr girl presents with a 2wk h/o asympt skin eruption. A wk before the onset, she had experienced mild fever, sore throat, & coryza for which she received symp Tx without any significant sequelae.

-A h/o similar complaints was also elicited in some of her school friends
-The lesions were not associated with any atopic diathesis, preceding inflammatory dermatoses, drug intake, or suggestive systemic finding.

-O/E, multiple discrete hypopigmented macules & patches of varying sizes (0.5-1.5 cm) without any surface change were seen to be distributed symmetrically involving the dorsal & ventral aspects of bilateral lower limbs

-A few similar scattered lesions were noted over the arms and trunk. Other mucocutaneous sites, including the palms and soles, were not affected. Examination of other organs systems were insignificant.

-A Woods lamp examination & KOH mount from skin scrapings ruled out pityriasis versicolor.
-Other laboratory investigations (including ASO titer, serological tests for HIV & viral hepatitis, SARSCOV2) were noncontributory
-Her family members refused consent for skin biopsy.

-Based on clinical findings, a diagnosis of eruptive hypomelanosis was established.
-She was prescribed a topical calcineurin inhibitor (tacrolimus 0.1% ointment) & emollient
-Near complete resolution was observed at the 2-month follow-up.

Eruptive hypomelanosis is a newly described paraviral exanthem that is attributed a to viral etiology owing to its programmed clinical course of prodromal symp, skin rash erupting as waves, occasional systematic symp (lymphadenopathy), & spontaneous remission approx 2-4 wks later

The cutaneous features are characterized by the sudden appearance of asymptomatic, well-demarcated hypopigmented macules chiefly distributed over extremities in the pediatric age group.
The DD's are

A New Concept: Paraviral Eruptions; Some acute rashes like this are difficult to classify within the established nosology.
