Not every patient with anemia needs blood and not is blood transfusion free of complications.

I'll list out a few TRANSFUSION REACTIONS in case you'll have forgotten about them ๐Ÿ‘‡๐Ÿป

Let's start w/ TACO๐ŸŒฎ

This isn't the good Mexican ๐ŸŒฎ we love๐Ÿคฃ

This is pulmonary oedema resulting from overzealous transfusion๐Ÿคทโ€โ™‚๏ธ

Risk factors: patients who can't handle fluids we'll, i.e. patients w/ kidney,heart disease etc

Totally avoidable complication๐Ÿ™๐Ÿป

A close differential of TACO is TRALI. The symptoms are the same but this is non-cardiogenic pulmonary oedema.

So BNP will be normal here, neck veins won't be dilated

TACO needs diuretics and some restraint on your part๐Ÿคฃ

TRALI=supportive care

These are bad, but the worst is acute hemolytic transfusion reactions ๐Ÿ˜ญ๐Ÿ˜ญ

Can be fatal if not recognised & managed immediately๐Ÿ˜ณ

Clinically: fever, dyspnoea, flank pain, red urine

All because of intravascular hemolysis & most often due to mismatched group transfusion๐Ÿ˜ญ

It's less evil twin is DELAYED HEMOLYTIC TRANSFUSION REACTION

This is d/t alloimmunization & is more often seen in heavily transfused pts.

Occurs a week or so after transfusion, is IgG mediated (rarely complement) so p/w features of EXTRAVASCULAR HEMOLYSIS !

There's another common reaction we see very frequently๐Ÿ˜ญ

FEBRILE NON-HEMOLYTIC TRANSFUSION REACTION (FNHTR)

This is d/t anti-leucocyte antibodies and is minimised if leucodepleted blood products are used.

P/w fever but no signs of hemolysis๐Ÿ™๐Ÿป

Needs some supportive care.

HIV
HTLV
CMV
Parvo

Also, every unindicated transfusion takes up valuable nursing time.

Blood banks work hard to give you the product, help them out by saying NO to inadvertent transfusions ๐Ÿ™๐Ÿป

As a physician I'm sure you've seen patients with ITP!

Let's start by calling it IMMUNE thrombocytopenia instead of IDIOPATHIC thrombocytopenia.

The P stood for Purpura in the past. Not anymore!

T= Thrombocytopenia = <100,000 platelets

If the thrombocytopenia lasts >12 months, the patient has chronic ITP ๐Ÿ˜ญ

This is unfortunately more common in adults; almost 75% develop chronic ITP ๐Ÿ˜ญ

Yeah, those are hemighost cells !

That's a hallmark of severe oxidative damage to red cells. Most often seen in G6PD deficient individuals!

I say most often because it can happen in patients with normal G6PD levels when the oxidative damage is severe ๐Ÿ˜ญ

You cannot see Heinz bodies in that image ๐Ÿ˜” They will NOT be seen on Romanowsky stains !!!

The red cell will appear like an emptied out "ghost cell" when the hemoglobin denatures ๐Ÿ˜” That's what you saw โ˜๐Ÿผ

๐Ÿ‘‡๐Ÿป Is a supravital stain showing Heinz bodies (blue dots)!

Since it's elevated in acute inflammation, it must be an inflammatory marker ๐Ÿคท๐Ÿปโ€โ™‚๏ธ

Yes, it is ๐Ÿ™Œ๐Ÿป

ฮฑ2 contains ceruloplasmin, haptoglobin and ฮฑ2 macroglobulin, all acute phase reactants!

Transplanter docs were using AZATHIOPRINE & CORTICOSTEROIDS to suppress the immune system ๐Ÿ˜ณ๐Ÿ˜ฎ

These drugs weren't good enough ๐Ÿ˜ญ

Time to talk about EOSINOPHILS today. Not as common a consult as thrombocytopenia and anemia but important nonetheless.

A short ๐Ÿงต to touch upon all things eosinophilia ๐Ÿ‘‡๐Ÿป

Before we go any further, it's important to know this

๐ŸฉธEosinophil โ‰ฅ 500 is ABNORMAL

Eosinophil is a predominantly tissue dwelling cell so ๐Ÿฉธ eosinophil DON'T correlate with tissue damage ๐Ÿคทโ€โ™‚๏ธ

We see a few patients with โคด๏ธโคด๏ธEos and no ๐Ÿซ€๐Ÿซ injury !!
Where do eosinophils come from?

Short answer: Bone Marrow

They are cells of the granulocytic lineage, siblings of neutrophils and basophils, children of the myeloblast (image๐Ÿ‘‡๐Ÿป)

IL5 is the cytokine responsible for it's production, important to know because we have anti IL5 mAb Image
