Covid (@UCSF) Chronicles, Day 13

1/ Little new @ucsfhospitals, which is great news. Covid+ pts=14–stable (last 5d: 11->14->13->13->14). 9 in ICU, no change over past few days. Re: ICU, relatively few new pts coming into ICU, but not many pts leaving either (see #6 for more ICU)
2/ Citywide: continued slow growth; I’ve included total #s and growth/day, both reassuring. Remarkably, on March 10, NYC and SF had ~same # of cases (14 vs 32). Today: NYC 38,087 vs SF 374. Deaths: 914 NY, 6 SF. Key message: can’t let up!
3/ NY scenes terrifying & upsetting. Remember: while 30 states have stay-at-home orders, 20 still don’t nyti.ms/3dETXfi Seems irresponsible. Figure: 2 nearby provinces in Italy, one did early shelter, other didn’t. Outcomes night & day (Fig thanks to @Rutherford_UCSF)
4/ @ucsfhospitals, we’re doing ok with our supplies: PPE, ventilators, beds. Testing capacity is good, but we’re still facing shortage of nasal swabs. Yes, still swabs. Hi tech meets low tech: several centers trying to 3D-print nasal swabs. Seems crazy, but that’s where we are
5/ We're starting to open up @ucsfhospitals for some urgent surgeries previously on hold – perhaps 20-30% of usual #. Best to stop calling them “elective” since folks think of plastic surgery or joint replacement. We’re talking about removing brain cancers & heart bypass surgery
6/ New NEJM study bit better re: ICU outcomes for pts on vent bit.ly/3bE0mWh Prior studies: China (at least 81% mortalty) bit.ly/2JDtGjX & WA (67%) bit.ly/2JtlSkG NEJM->50% mortalty (12/24, tho 3 pts still on vent). Not gr8, but says intubatn not futile
7/ Happy Doctor’s Day! Heroes are my colleagues, @UCSF & everywhere, who are taking care of patients under these extraordinary conditions, at some personal risk (I’m not on clinical now, so not me). Today’s @NewYorker cover captures the moment movingly.

Let’s talk California. Until recently, our state managed to do so well that our experience has been dubbed The California Miracle. But let’s say it clearly: The Miracle is over. The sooner we acknowledge that, the better off we’ll be.
2/ In many of the surging states, including TX, AZ, and FL, one can easily point to leadership malpractice by governors and other politicians. They doled out misinformation about Covid and failed to require evidence-based practices, like masking, to prevent spread. Unforgivable.
3/ California is different: our leaders acted responsibly. And Californians mostly did too.

But around Memorial Day, standards were relaxed, people (yes, mostly young ones) got complacent, and – after a string of lucky breaks – our luck ran out. Now it’s our time.

This week, it felt like we entered a new phase of Covid. In March & April, we had panic, lockdowns, and surges… in certain places. The next phase (May-early June) saw cases & deaths plateau, we began opening up…and the seeds of complacency.
2/ And now we’ve entered another phase, with major surges in places that were spared the first time around. Given all we’ve learned in the past few months, it’s all very predictable, and it was at least partly preventable – which makes it especially sad.
3/ Predictable, because the best evidence says that the virus’s essential characteristics – its ease of transmission and virulence – are unchanged. And, unless you’re living in New York (where 15-20% of people have antibodies), the virus's targets – namely, us – are the same too.

Great UCSF Grand Rounds today: bit.ly/2Z9eOB9 I interview John Barry, author of "The Great Influenza: The Story of the Deadliest Pandemic in History", now #1 nonfiction paperback @nytimes. Watch if you can (65 min); summary below.
2/ Brief local update first: For first time since March, some worrisome signs in SF, though not nearly as dire as TX, AZ, FL. (SoCal’s not great either.) @UCSFHospitals: 13 pts, 4 vented. Fig shows “W” pattern over last 2 wks, not surging but definite bump since early June.
3/ SF: cases also up (36/d, vs 23/d last wk; Fig L), & small bump in hospital pts (Fig R). A bit misleading since some are pts transferred from two outbreaks elsewhere: Imperial County in SoCal & nearby San Quentin prison bit.ly/2ZcN7XY (w/ >500 infected, incl 51 staff)
