1/3 I’m hearing anecdotal reports that certain young, healthy but potentially higher risk patients are getting sick with #COVID19 and doing bad. @CDCgov must urgently publish the collective U.S. clinical experience to guide care. China was far ahead at this.
2/3 Doctors are being forced to let clinical experience guide care decisions in absence of systematic data. They’re experimenting with fluids, high flow oxygen, prone positioning - in absence of systematic literature and data on what works and what doesn’t. This isn’t acceptable.
3/3 Publications won’t matter much in May. They’re needed tomorrow. Today. Urgently. What is working in the ICUs? I’m hearing mortality for intubated patients is running 50-80%, extubation takes 10-14 days. What is it? Get doctors data that will optimize care. Get it today.

THREAD: In our report on steps to reopen America, we reference the need to be able to test at least 750,000 people a week. That number is being discussed. Here is more clarification on how we derived it, and what it means. 1/n aei.org/research-produ…
750,000 is the approximate number of U.S. medical visits per week for influenza like illness during peak flu season. We use it as estimate of what’s necessary in setting where #covid19 isn’t yet circulating, and we want to detect furtive spread. That is not the situation now. 2/n
Right now, #covid19 is epidemic. And even when we’re likely to seek to reopen parts of America in May and June, there will be baseline spread. The epidemic won’t simply collapse. Against that backdrop, we need much more test capacity to safely make transition to normal life 3/n

THREAD: There’s uncertainty how many Americans were already exposed to #COVID19 and developed immunity. Until we do widespread serological testing to see who has antibodies, we won’t know for sure. Experts, models put number in single digits, perhaps 2-5% of U.S. population. 1/n
The excellent Trevor Bedford of the Hutch had a good thread on this yesterday. I’ll recap - and build on - some of his characteristically insightful points. 2/n
Serology (testing for antibodies people produce after being exposed to #COVID19) tells you where you’ve been. It doesn’t tell you where you’re going. In other words, it reveals where infection has spread. But doesn’t diagnose active infection or tell you where it’s spreading. 3/n

1/n There's some concerns being expressed about the implications for privacy and liberty of case based interventions that encourage widespread screening for #COVID19, and use public health interventions that require people who are infected and contagious to self isolate.
2/n These concerns shouldn't be ignored, but also should be put in context. Case-based interventions that identify disease outbreaks, and contain spread, are a mainstay of public health prevention for deadly pathogens from Polio to Ebola to Measles, and countless deadly pathogens
3/n How we implement these efforts, and seek consent, should be carefully considered. But ultimately, we face hard trade-offs between pervasive risk of epidemics and outbreaks; population mitigation (what we have now); or the effective application of case-based interventions.
