1) Why are you so behind?
2) Why did it take a crisis to help you start basic changes?
3) What are you going to do to fix these long running problems going forward.
... oh, and stop clapping yourself on the back for stuff you should have done long ago.

Personally, I'd start with looking at your company doctrine. Take the following table and colour it in (two examples given). If you're more red than green ... fix that doctrine fast.
