(3) Could the spread of the virus be related to a river watershed?

Based on current theories of transmission we wouldn't expect that, other than a river creating a wind current perhaps.

What about the fecal-oral route?
(4) A framework has been developed for understanding ways fecal matter contaminated with a virus can come back in contact with humans and be ingested. Insects and animals may play a role.

(5) Maybe those high tech Japanese toilets played a role in controlling transmission.
(6) In summary, the outbreak in Italy was largely contained to the Po river watershed.

The Apennines may have provided a natural barrier of some sort, protecting Southern Italy or the watershed itself may play a role in transmission.

Food for thought.
