Want to show you something interesting

Now, most of my type 2 diabetes patients are on Acarbose.

Recently a patient was mine was not placed on Acarbose. He was on basal insulin with other OADs. I want to show you his AGP...
His AGP is very interesting. Look at the post meal excursions on a patient not on Acarbose.

Eventhough his Time in Range is good, high glycemic variablity is pretty high Image
This is his data from @Tidepool_org

His Time in range is 68% which is not too bad, but his SD is 45 mg/dl (I would like that in the 30's in a patient with mean glucose of 110 mg/dl) and his Coefficient of variation is 41% (should be less than 33%) Image
The same day another patient showed up. Similar profile. He was also put on Basal insulin + OAD, but in his case we used Acarbose.

Look at his AGP data. Very little intraday variablity. So simply dont see those post meal peaks Image
Now look at the @Tidepool_org data of Patient 2

Similar Average glucose value and GMI, but higher time in range and lower glycemic variablity Image
Despite the minor problems like flatulence etc, I just thing Acarbose works. It just work so well in Indian type 2 diabetes patient.

MC Thomas in his article argues that Type 2 Diabetes should be renamed the "CARAMEL Disease" (A 🧵)
What does CARAMEL stand for ?

CARAMEL stands for CArdiac, Renal, Adipo-Metabolic, Eye and Liver disease
His argument is that Type 2 diabetes is a mere manifestation of the "actual disease". So are the other manifestation of the CARAMEL acronym which all arise from the common soil... So that this common soil?

Q. What is Hemoglobin Glycation Index ?
1/5: 🩸 The Hemoglobin Glycation Index (HGI) compares a person’s actual HbA1c level to their predicted level based GMI (glucose management index) value from CGM
2/5: 📊 The HGI is calculated by subtracting the estimated HbA1c from the measured HbA1c. A high HGI suggests increased glycation rate, linked to microvascular complications in diabetes.

What is the Clarke error grid?

The Clarke Error Grid Analysis (EGA) is a tool developed in 1987 to assess the clinical accuracy of blood glucose meters by comparing their readings to reference values.
The grid breaks down a scatterplot of a reference glucose meter and an evaluated glucose meter into five regions:
Region A are those values within 20% of the reference sensor,
